彩超诊断先天性双孔二尖瓣畸形1例并文献复习

摘  要:目的通过分析双孔二尖瓣畸形的彩色多普勒超声表现,加深对此少见病的认识,提高诊断水平。方法总结1例经手术证实的双孔二尖瓣畸形病例,并查阅相关中外文献,详细分析其声像图表现。结果患者以心悸、胸闷为首发症状,超声表现:心脏舒张期左室短轴二尖瓣水平切面显示二尖瓣开放时呈两个分离、左右排列的瓣口,形似"眼镜"样;心脏收缩期左房内可见两束返流。结论彩色多普勒超声是发现和诊断双孔二尖瓣畸形的重要手段,同时还可观察瓣膜功能、检出是否合并其他畸形或继发性改变,为临床治疗方案的选择提供可靠地依据。Objective To analyze Color Doppler ultrasonic features on detection of double orifice mitral,so as to deepen the understanding of this unusual disease and to improve the diagnostic level at the stage.Methods One case of operative diagnosed double orifice mitral valve was summarized.Lots of relevant Chinese and foreign literatures were searched.The ultrasonic performance of the case was analyzed.Results The very first symptoms included palpitation and chest distress.Ultrasonic performance showed that during diastole,left ventricular short axis of mitral valve revealed two valve orifices arranged in left and right,like a pair of glasses;during systole,two beams were observed in left atrium.Conclusion Color Doppler ultrasonic examination is one of the important methods for diagnosis of double orifice mitral valve and observation of the function of valve.We will provide valuable evidence to select treatments.
