摘  要:目的检测临床孕妇外周血标本中49种常见氨基酸与酰基肉碱的浓度,分析主要妊娠疾病的氨基酸、酰基肉碱的表达水平。方法选取2015~2016年广州医科大学附属第三医院产科收治的78例孕妇病例资料作为研究对象,按照是否合并妊娠期疾病分为正常对照组31例、妊娠疾病组47例。妊娠疾病组又分为妊娠期糖尿病组(GDM组,16例)、妊娠期高血压疾病(HDP组,20例)和GDM+HDP组11例,利用液相串联质谱技术检测分析18种氨基酸与31种酰基肉碱表达水平,并进行统计学分析。结果各组检测的氨基酸及酰基肉碱水平比较,有12项指标组间有显著性差异(P<0.05),包括甘氨酸(Gly)、谷氨酰胺(Gln)、色氨酸(Trp)、丙酰基肉碱(C3)、丙二酰基肉碱/3-羟基丁酰肉碱(C3DC/C4OH)、丁酰基肉碱(C4)、异戊酰基肉碱(C5)、己酰基肉碱(C6)、己二酰基肉碱(C6DC)、辛烯酰基肉碱(C8∶1)、十四烷酰基肉碱(C14)、3-羟基十六烯酰基肉碱(C16∶1OH)。其中Gly、Trp、C3、C3DC/C4OH、C4、C5、C6、C6DC在正常对照组的表达水平显著低于妊娠疾病组(P<0.05)。结论妊娠疾病可以导致体内多种氨基酸和酰基肉碱的代谢水平表达差异,可能与其影响氨基酸与酰基肉碱的代谢通路有关。相关分子机制有待于进一步深入研究探讨。A total of78pregnant women from Obstetrics Department of the Third Affiliated Hospital of Guangzhou Medical University were collected,and divided into control group(n=31)and pregnancy diseases group(n=47)which divided into gestational diabetes(GDM)group16patients,gestational hypertension(HDP)group20patients,and GDM+HDP group11patients.The tandem mass spectrometry was used for detecting18kinds of amino acids and31kinds of acylcarntine.The results were compared among the patients.Results:Twelve indicators were significantly different at the levels of amino acids and acylcarnitins in each group(P<0.05),including Gly,Gln,Trp,carnitine(C3),malonyl carnitine/3-hydroxybutyryl carnitine(C3DC/C4OH),butyryl carnitine(C4),isovaleryl carnitine(C5),caproyl carnitine(C6),hexanoylcarnitine cdipicotinyl carnitine(C6DC),octenoyl carnitine(C8∶1),myristoyl carnitine(C14),3-hydroxyhexadecanoylcarnitine(C16∶1OH).The expression levels of Gly,Trp,C3,C3DC/C4OH,C4,C5,C6and C6DC in the control group were significantly lower than those in the pregnancy diseases group(P<0.05).Conclusions:Pregnancy diseases can lead to abnormal metabolic level and expression of amino acids and acylcarnitine in vivo,which is related to the metabolism pathway of amino acids and acylcarnitine.The related molecular mechanisms should be further studied and verified.
