摘 要:目的探讨胆管癌组织中Cullin 4A(CUL4A)、血管内皮生长因子(VEGF)和N-钙黏蛋白(N-cadherin)的表达及其临床意义。方法利用实时荧光定量PCR(qRT-PCR)技术和免疫组织化学方法检测CUL4A、VEGF及N-cadherin在胆管癌及正常胆管组织中的表达情况,分析各指标与临床病理参数之间的关系,并对三指标进行相关性分析。结果 qRT-PCR结果显示,胆管癌组织中CUL4A m RNA表达率明显高于正常胆管组织(P<0.001)。免疫组化结果分析显示CUL4A、VEGF和N-cadherin蛋白在胆管癌组织和正常胆管组织中的阳性表达率分别为70.8%vs.2.8%(P<0.05);66.7%vs.8.3%(P<0.05);58.3%vs.12.5%(P<0.05)。CUL4A、VEGF和N-cadherin阳性表达均与肿瘤分化程度、血管侵犯、神经浸润、淋巴结转移及TNM分期有关(P<0.05)。CUL4A与VEGF、N-cadherin在胆管癌组织中阳性表达均呈正相关(P<0.05)。生存分析表明,CUL4A可作为胆管癌预后的一个独立性因素。结论 CUL4A、VEGF及N-cadherin与胆管癌血管生成及侵袭转移密切相关,CUL4A有望成为胆管癌诊断和靶向治疗的一个新基因。Objective In order to explore the expressions of CUL4A,VEGF and N-cadherin in cholangiocarcinoma and their clinical significance.Methods The expression of CUL4A,VEGF and N-cadherin in cholangiocarcinoma tissues and normal bile duct tissues was detected by quantitative real-time PCR(qRT-PCR)and immunohistochemistry.Results The expression of CUL4A mRNA was clearly up-regulated in 35 cholangiocarcinoma tissues compared to those in 15 normal bile duct tissues(P<0.001).The positive rate of CUL4A,VEGF and N-cadherin protein in cholangiocarcinomas was significantly higher than normal bile duct tissues(70.8%vs.2.8%,P<0.05),(66.7%vs.8.3%,P<0.05),(58.3%vs.12.5%,P<0.05),respectively.The differences of the expression of CUL4A,VEGF and N-cadherin proteins in cholangiocarcinoma of differentiation,vascular invasion,neural invasion,lymph node metastasis and clinical TNM stage were all significant(P<0.05).Significantly positive correlation was found between CUL4A and VEGF,N-cadherin by using spearman correlation analysis(all P<0.05).There was positive correlation between N-cadherin and VEGF(P<0.05).Survival analysis affirmed that CUL4A was an independent prognostic factor for cholangiocarcinoma.Conclusions The abnormal expression of CUL4A,VEGF and N-cadherin gene may promote tumor angiogenesis and progression of cholangiocarcinoma.CUL4A might be a potential target gene for the diagnosis and treatment in cholangiocarcinoma.
