摘  要:目的探讨中等强度有氧运动(MIAE)对表阿霉素(EPI)致大鼠心肌损伤的保护作用。方法将40只SD雌性大鼠随机分为正常对照组8只、心肌损伤组32只,建立EPI致大鼠心肌损伤模型,造模过程中有4只大鼠死亡。造模成功后,心肌损伤组28只大鼠进一步分为模型组(按3ml/kg剂量给予0.9%氯化钠溶液,1次/周)、右丙亚胺(DEX)组(按30mg/kg剂量给予DEX注射液,1次/周)、MIAE组(经3d适应性训练后,每周在同一时间训练大鼠,第1周按15m/min训练25min,随后逐渐增加运动强度,至第6周按24m/min训练30min,1次/d,5d/周)、DEX+MIAE组,每组7只,共治疗6周。检测各组大鼠血清心肌肌钙蛋白(TNNI3)、B型钠尿肽(BNP)水平,并剖腹取大鼠心室心肌组织进行病理学检查。结果治疗6周后各组大鼠血清TNNI3、BNP水平比较,差异均有统计学意义(均P<0.05);其中正常对照组低于模型组、DEX组、MIAE组、DEX+MIAE组(均P<0.05),DEX组、MIAE组、DEX+MIAE组低于模型组(均P<0.05),DEX+MIAE组低于DEX组、MIAE组(均P<0.05);DEX组与MIAE组比较差异无统计学意义(P>0.05)。HE染色及电镜检查结果显示DEX+MIAE组心肌损伤程度最轻,DEX组次之,MIAE组较为严重。结论 DEX、MIAE、DEX+MIAE对EPI所致大鼠心肌损伤均有心肌保护作用,以DEX+MIAE最为明显,但治疗后仍存在不同程度的心肌损伤。Objective To explore the effect of moderate-intensity aerobic exercise(MIAE)on myocardial damage induced by epirubicin in rats.Methods The myocardial damage model was induced by intravenous injection of epirubicin(EPI).The rats with myocardial damage were randomly divided into 4 groups with 7 in each group:rats in model group without any treatment,rates in dexrazoxane(DEX),MIAE and dexrazoxane+moderate-intensity aerobic exercise(DEX+MIAE)groups were treated correspondingly for 6 weeks,and 8 normal SD rats served as control group.The serum TNNI3 and BNP levels were measured and the ventricular tissue was pathologically examined in all groups.Results Six weeks after intervention,serum TNNI3 and BNP levels in the normal control group were lower than those in other 4 groups,in each treatment group was lower than those in model group,in DEX+MIAE group were lower than those in DEX group and the MIAE group(P<0.05).There was no significant difference between the DEX group and the MIAE group(P>0.05).Light microscopy with HE stain and electron microscopy show that there was myocardial injury in the model group,and the myocardial damage was attenuated in all treatment group and that was mild in DEX+MIAE group.Conclusion Dexrazoxane and moderate-intensity exercise can protect rats from myocardial damage induced by epirubicin,and the combination of two is more effective.
