摘 要:目的研究健康成年人脑灰质、白质体积和脑脊液含量随年龄的变化特点。资料与方法对124例右利手健康成年人(18~70岁)进行MR矢状位三维结构T1WI图像扫描,运用基于体素的形态学分析(VBM)技术获得全脑的灰质体积、白质体积、脑脊液含量。比较不同年龄组(18~30岁、31~40岁、41~50岁、51~60岁、61~70岁)受检者灰质体积的差异,分析脑灰质体积、白质体积及脑脊液含量随年龄的变化特点。结果脑灰质体积的萎缩分布具有不均匀性及时序性,扣带回、额叶先萎缩,逐渐扩展至双侧颞叶、双侧顶叶、双侧岛叶、双侧楔前叶、左侧小脑半球;全脑灰质体积随年龄呈线性下降,全脑白质体积随年龄呈二次方相关改变模式,脑脊液含量随年龄呈线性下降。18~70岁健康成人灰质体积千分比为(453±22)‰,白质体积千分比为(369±18)‰,脑脊液含量千分比为(178±23)‰。结论 VBM技术能够敏感地测量全脑灰质、白质和脑脊液体积随年龄变化的时间、空间模式,有助于进一步了解大脑退化的规律。Purpose To study the age-related changes of cerebral gray matter, white matter volume and cerebralspinal fluid content in healthy adults. Materials and Methods MRI sagittal three-dimensional structural T1WI images scanning was performed on the 124 dextral healthy adults (aged 18 to 70). Voxel-based morphometry (VBM) analysis was taken to obtain gray matter volume, white matter volume and cerebralspinal fluid content of the whole brain. Difference of gray matter volume among different age groups (18-30 years, 31-40 years, 41-50 years, 51-60 years, 61-70 years) was compared and changes of cerebral gray matter volume, white matter volume and cerebralspinal fluid content along with age were analyzed. Results Atrophy distribution of cerebral gray volume was imbalanced and of time sequence. Cingulate gyrus and frontal lobe withered firstly and then gradually expanded to bilateral temporal lobe, bilateral parietal lobe, bilateral insular lobe, bilateral precuneus and left cerebellar hemisphere. Gray matter volume of whole brain decreased linearly with age and white matter volume of whole brain was in quadratic changes with age. Cerebralspinal fluid content decreased linearly with age. Gray matter volume per mil of 18-70 healthy adults was about (453±22)‰, that of white matter volume was about (369±18)‰ and that of cer ebralspinal fluid content was (178±23)‰. Conclusion VBM technology can sensitively measure the temporal and spatial patterns of whole brain gray matter, white matter and cerebrospinal fluid volume with age, and help to further understand the laws of brain degradation.
