摘 要:目的探讨Stathmin及P16在宫颈鳞状上皮病变诊断中的意义。方法运用组织芯片技术及免疫组织化学技术对177例宫颈鳞状上皮病变组织(浸润性鳞状细胞癌87例、高级别鳞状上皮内病变60例:26例CIN2,34例CIN3、低级别鳞状上皮内病变30例)及对照组正常宫颈组织30例中Stathmin及P16蛋白的表达进行检测。结果 Stathmin在宫颈浸润性鳞状细胞癌、宫颈高级别鳞状上皮内病变、宫颈低级别鳞状上皮内病变及对照组正常宫颈鳞状上皮中的阳性率分别为95.40%(83/87)、65.00%(39/60)、3.33%(1/30)、0(0/30),其中CIN2与CIN3的阳性率分别为34.62%(9/26)、88.24%(30/34)。P16在宫颈浸润性鳞状细胞癌、宫颈高级别鳞状上皮内病变、宫颈低级别鳞状上皮内病变及对照组正常宫颈鳞状上皮中的阳性率分别为98.85%(86/87)、93.33%(56/60)、10.00%(3/30)、0 (0/30),其中CIN2与CIN3的阳性率分别为92.30%(24/26)、94.12%(32/34)。浸润性鳞状细胞癌及高级别鳞状上皮内病变组织中,Stathmin与P16的阳性率均显著高于低级别鳞状上皮内病变及正常宫颈鳞状上皮组织(P均<0.05)。在CIN2与CHIN3中,Stathmin的差异有统计学意义(<0.05),而P16无差异。结论Stathmin及P16可能成为宫颈高级别鳞状上皮内病变及宫颈鳞状细胞癌的一个辅助诊断指标,且Stathmin在需要鉴别CIN2与CIN3时有一定的诊断价值。Objective To investigate the The diagnostic value of Stathmin and P16 in the cervical squamous lesions. Methods Using the tissue microarray and immunohistochemical to detect the expression of Stathmin and P16 in the 177 cervical squamous lesions(87 squamous cancer、60 high grade squamous intraepithelial neoplasia:26 CIN2,34 CIN3、30 low grade squamous intraepithelial neoplasia) and the control group 30 normal cervical epithelial. Results The positive rates of Stathmin in the squamous cancer、high grade squamous intraepithelial neoplasia、low grade squamous intraepithelial neoplasia and normal cervical epithelial are 95.40%(83/87),65.00%(39/60),3.33%(1/30) and 0(0/30) respectively,the positive rates of Stathmin in CIN2与CIN3 are 34.62%(9/26)、88.24%(30/34) respectively. The positive rates of Stathmin is significantly higher in the squamous cancer、high grade squamous intraepithelial neoplasia than in the low grade squamous intraepithelial neoplasia and normal cervical epithelial (P<0.05). And The positive rates of Stathmin are significantly difference between the CIN2 and CIN3. The positive rates of P16 in the squamouscancer、high grade squamous intraepithelial neoplasia、low grade squamous intraepithelial neoplasia and normal cervical epithelial are 98.85%(86/87),93.33%(56/60),10.00%(3/30) and 0(0/30)respectively. The positive rates of P16 in CIN2与CIN3 are 92.30%(24/26)、94.12%(32/34) respectively. Conclusion Stathmin and P16 are the auxiliary function in diagnosis of high grade squamous intraepithelial neoplasia and cervical squamous cancer,and Stathmin maybe has certain diagnostic value in the identification of CIN2 and CIN3.
