摘  要:目的探讨胸腹腔积液细胞蜡块技术在明确肿瘤来源及在肺腺癌表皮生长因子受体(EGFR)检测中的应用价值。方法收集2016年6月至2017年12月于克拉玛依市中心医院住院并送检胸腹腔积液的312例患者中细胞量较多的104例制作细胞蜡块,并进行常规HE染色、免疫细胞化学染色、表皮生长因子受体(EGFR)突变检测。结果104例患者中,通过细胞蜡块方法明确查见肿瘤细胞81例,此81例患者通过免疫组织化学染色方法进一步明确肿瘤组织来源者72例;运用实时荧光定量聚合酶链式反应方法,对27份肺腺癌标本进行EGFR基因突变检测,结果显示12例有EGFR基因突变。结论胸腹腔积液细胞蜡块技术借助免疫组织化学染色方法有助于明确诊断肿瘤性质及组织来源,也能成功进行肿瘤基因突变检测,为患者的分子靶向治疗提供依据。Objective To investigate the value of hydrothorax and seroperitoneum cell blocks in identification of tumor source and detection of epidermal growth factor receptor (EGFR) gene of lung adenocarcinoma. Methods One hundred and four samples with most cells from 312 cases of hydrothorax and seroperitoneum checked in Karamay Central Hospital from June 2016 to December 2017 were selected to make cell blocks. And then, they were collected for conventional HE staining, immunohistochemical staining and EGFR mutation detection. Results Neoplasm was found in 81 of 104 cell blocks. Seventy-two of 81 cases were identified the origin of the neoplasm by immunohistochemical staining. EGFR gene was detected in 12 cases of 27 tissue blocks of lung adenocarcinoma specimens by real-time quantitative polymerase chain reaction. Conclusions Hydrothorax seroperitoneum cells block technology combined with immunohistochemistry has significant value in the differential diagnosis of neoplasm and gene mutation detection, which can provide reference to individualized molecular targeted therapy.
