摘  要:目的探讨问题教学法(PBL)联合案例教学法(CBL)在本科生妇科超声学习中的应用。方法选择2017年9至12月在西安交通大学第一附属医院妇产科超声监护室见习的临床医学专业五年制学生共74名为研究对象,根据不同的教学方法分为两组,对照组采用PBL教学方法,观察组采用PBL联合CBL教学方法,通过调查问卷、临床考核综合评价两组学生的教学效果。结果观察组学生自我评价总分显著高于对照组(t=3.342,P<0.05),激发学习兴趣、提高临床思维能力、提高对妇科常见病的诊断能力、有助于基础理论知识的理解、对教学方法满意各分项得分均显著高于对照组(t值分别为2.908、2.976、2.953、3.014、3.358,均P<0.05)。观察组学生考试总成绩显著高于对照组(t=2.658,P<0.05),其中病例分析题考试成绩显著高于对照组(t=2.994,P<0.05),理论知识题考试成绩两组间比较无显著性差异(t=1.035,P>0.05)。观察组学生考试优秀率为68.42%,对照组为36.11%,Fisher确切概率法检验显示观察组显著高于对照组(P=0.020)。结论 PBL联合CBL教学法有助于提高本科生妇科超声学习带教质量,值得推广应用。Objective To explore the application of problem-based learning (PBL) combined with case-based learning (CBL) teaching method in gynecological ultrasonic study of undergraduates.Methods A total of 74 undergraduates,majoring in clinical medicine and practicing in the First Affiliated Hospital of Xi'an Jiaotong University from September to December 2017,were selected.These students were divided into two groups according to different teaching methods.Students in control group accepted PBL teaching method,and students in observation group accepted PBL combined with CBL teaching method.The teaching effect of students was comprehensively evaluated by questionnaire and clinical assessment.Results The self assessment score in the observation group was significantly higher than that in the control group ( t =3.342, P <0.05).Scores of items including stimulating interest in learning,improving clinical thinking ability,improving diagnostic ability of gynecological common diseases,contributing to the understanding of basic theoretical knowledge,being satisfied with the teaching method in the observation group were significantly higher than those in the control group ( t value was 2.908,2.976,2.953,3.014 and 3.358,respectively,all P <0.05).The total test scores of students in the observation group were significantly higher than those in the control group ( t =2.658, P <0.05),and the scores of case analysis were also significantly higher ( t =2.994, P <0.05).There was no significant difference in theoretical knowledge questions between two groups ( t =1.035, P >0.05).The test excellence rate of the observation group was 68.42%,which was significantly higher than that (36.11%) of the control group analyzed with Fisher's exact probability test ( P <0.05).Conclusion PBL combined with CBL teaching method is helpful to improving the quality of gynecological ultrasound teaching and learning for undergraduates,which is worthy of promotion and application.
