摘  要:目的探讨增强T1WI直方图分析鉴别原发性中枢神经系统淋巴瘤(PCNSL)和胶质母细胞瘤(GBM)的价值。方法回顾性分析经手术病理证实的41例PCNSL和30例GBM。收集2种肿瘤的T1WI、T2WI和增强T1MR图像,用软件MaZda提取肿瘤的ROI,针对肿瘤最大层面的ROI进行直方图分析,观察2种肿瘤的直方图特征,包括均值,方差,偏度,峰度,第1、10、50、90、99百分位数,找出2种肿瘤之间的显著性差异。结果利用直方图分析提取出的9个参数特征中,均值、方差、偏度、第1百分位数、第10百分位数及第50百分位数差异有统计学意义(P均<0.05),峰度、第90百分位数及第99百分位数差异无统计学意义(P均>0.05)。在这些参数中,偏度鉴别PCNSL和GBM的效能最高,AUC为0.960,敏感度和特异度分别为80%、100%。结论直方图分析可提供更多量化信息参数特征,其中偏度具有较高诊断效能,为PCNSL和GBM的鉴别诊断提供了新的思路和方法。Objective To investigate the value of enhanced TWI histogram analysis in the identification of primary central nervous system lymphomas(PCNSL)and glioblastomas(GBM).Methods 41 cases PCNSL and 30 cases of GBM confirmed by pathology were analyzed retrospectively in our hospital.The T1WI,T2WI,and enhanced T1MR images of the two groups were collected,and the MaZda was used to extract the region of interest ROI of the tumor.The histogram analysis was performed on the maximal region ofinterest of the tumor.The histogram features of the tumors,including the mean,variance,skewness,kurtosis,and the 1,10,50,90,and 99 percentiles,were used to find significant differences between the two groups.Results Among the 9 parameters extracted from histogram analysis,the mean,variance,skewness,1st,10th,and 50th percentiles were statistically significant(P<0.05).There was no significant difference in kurtosis,90th percentile,and 99th percentile(P>0.05).A mong these parameters,skewness was the most effective in identifying PCNSIand GBM.The area under the ROC curve was 0.960,while the sensitivity and specificity were 80%and 100%,respectively.Conclusion The histogram analysis can provide more information about quantitative parameters.Among them,the skewness has high diagnostic efficiency,and provides new ideas and methods for differential diagnosis of PCNSI and GBM in the central nervous system.
