摘  要:目的探讨低强度经颅超声(LIPUS)对大鼠永久性大脑中动脉栓塞(dMCAO)后脑水肿组织的水通道蛋白 4(AQP4)表达及相对表观扩散系数(rADC)变化的影响。材料与方法将 40 只雄性 SD 大鼠随机分成 4 组,每组 10 只,建立 dMCAO模型。对照组(A 组)不给予超声刺激,实验组根据脑梗死分期分为超急性早期、超急性晚期和急性期,分别于建模后 1 h、3 h、9 h 给予超声刺激(分别记为 B、C、D 组),刺激 10 min 后进行 MRI 扫描,测量梗死区及对侧镜像区 ADC 值,并计算rADC 值,随后取脑,进行免疫组织化学染色,对病变脑组织进行病理观察,AQP4 免疫组织化学染色半定量结果以阳性细胞百分比表示。结果 A、B、C、D 组的 AQP4 阳性细胞百分比分别为 87%、67%、65%、88%。建立 dMCAO 模型后 12 h,A、B、C、D 组大鼠脑 rADC 值分别为 0.24±0.15、0.39±0.22、0.42±0.29 及 0.23±0.18,B 组、C 组明显高于 A 组(P=0.016、0.018),D 组与 A 组比较差异无统计学意义(P=0.850)。结论 AQP4 表达及 rADC 变化可以反映 LIPUS 治疗对大鼠超急性期及急性期脑梗死脑水肿的变化。Purpose To investigate the effect of low intensity pulsed transcranial ultrasound (LIPUS) on the expression of aquaporin 4 (AQP4) and the relative apparent diffusion coefficient (rADC) of cerebral edema after arterial embolization in rats. Materials and Methods Forty male SD rats were randomly divided into 4 groups (n=10) to establish a permanent distal middle cerebral artery occlusion (dMCAO) model. The control group (group A) was not given the ultrasound stimulation. According to the stage of cerebral infarction, the experimental group was given the ultrasound stimulation at 1 h, 3 h, and 9 h after modeling (recorded as group B, group C and group D). The ADC values of the infarct area and the opposite side image area were measured, and the rADC value was calculated. Then the brain was taken from the rats for immunohistochemical staining. The pathological observation was made on the lesion brain tissue. Semi-quantitative results of AQP4 immunohistochemical staining were expressed as percentage of positive cells. Results The percentage of AQP4 positive cells in group A, B, C and D were 87%, 67%, 65% and 88%, respectively. At 12 h after the dMCAO modeling, the rADC values of the rat brains in group A, B, C and D were 0.24±0.15, 0.39±0.22, 0.42±0.29 and 0.23±0.18, respectively. The values of group B and group C were significantly higher than those of group A (P=0.016 and 0.018), and there was no significant difference between group D and group A (P=0.850). Conclusion AQP4 expression and change to rADC can reflect the change to cerebral edema of hyperacute and acute cerebral infarction in rats treated by LIPUS.
