摘 要:目的分析慢性化脓性中耳炎静止期人群在噪声环境下的听力变化趋势,结合耳鼻咽喉科临床病例分析,为完善职业病诊疗规范提供支持。方法选择中山市疾病预防控制中心门诊部2018年3月至2019年1月间体检者中23例慢性化脓性中耳炎静止期且符合GBZ 188-2014规定不属于噪声职业禁忌证者为实验组,另选择与实验组处于相同噪声环境、同一性别、同一年龄矫正值的年龄段、中耳结构正常、纯音听阈测试听力正常的体检者23例为对照组。对两组受试者进行纯音听阈测试,比较6个月前及6个月后的两次纯音听阈测试结果。每人按时间前后顺序任意一个频率听阈增高≥5 d B即视为听力发生变化。统计慢性化脓性中耳炎静止期人群和耳结构正常人群纯音听阈测试变化的例数,进行描述性分析。结果实验组共9例存在纯音听阈变化,对照组共11例存在纯音听阈变化,两组比较差异无统计学意义(P>0.05);实验组与对照组在0.5 kHz、1 kHz、2 kHz、3 kHz、4 kHz、6 kHz、频率气导、骨导听阈变化值间比较差异均无统计学意义(P>0.05)。结论慢性化脓性中耳炎静止期人群和耳结构正常人群在噪声环境中听力下降变化趋势无明显差异。Objective To analyze the trend of hearing changes in chronic suppurative otitis media(CSOM in stationary phase under noise environment, and to provide support for improving the diagnostic and therapeutic criteria of occupational diseases by analyzing the clinical cases of otorhinolaryngology. Methods From March 2018 to January 2019, 23 patients with chronic suppurative otitis media in stationary phase who met the GBZ 188-2014 regulations and did not belong to the occupational contraindication of noise in Zhongshan Center for Disease Control and Prevention were selected as the experimental group. Another 23 healthy subjects with the same noise environment, gender,age corrected value, normal middle ear structure and normal pure tone hearing threshold test were selected as the control group. Pure tone hearing threshold test was performed in both groups, and the results of the tests before and after 6 months were compared. Hearing changes were regarded as the hearing threshold increased by no less than 5 dB at any frequency in the order of time. The number of cases of pure tone audiometric threshold changes in stationary phase of chronic suppurative otitis media and those with normal ear structure were counted, and the descriptive analysis was carried out. Results There were 9 cases of pure tone hearing threshold changes in the experimental group and 11 cases in the control group. There was no significant difference between the two groups(P>0.05). There was no significant difference between the experimental group and the control group in the changes of 0.5 kHz, 1 kHz, 2 kHz, 3 kHz, 4 kHz, 6 kHz, frequency air conductance, and bone island hearing threshold(P>0.05). Conclusion There is no significant difference in the trend of hearing loss between the people of chronic suppurative otitis media in stationary phase and the people with normal ear structure under noise environment.
