摘 要:目的探讨营养评价精法(MNA-SF)用于养老院老人营养状况评价的实用性及有效性,为养老院简便发现老年人营养不良和及时进行干预提供依据。方法采用MNA-SF及控制营养状态评分(CONUT)对来自养老院的老年科门诊及病房患者营养状况进行调查,分析两者的相关性及一致性。结果从用时看,COUNT法无法当天获得结果,而MNA-SF法仅需4 min;从结果看,COUNT法测得的营养正常占比39.3%,轻度、中度、重度营养不良分别为30.4%、23.7%、6.6%,MNA-SF法测得的营养正常比例为36.6%、营养不良占比63.4%;比较来看,MNA-SF和COUNT相关系数(r=-0.863,P<0.01);与COUNT相比,MNA-SF法的灵敏度为80.2%,特异性为89.7%。结论养老院中老年人营养不良发生率高,MNA-SF是一种简单、易行、实用的营养状况筛查工具,与临床中和疾病相关的营养评价方法有较高的一致性。Objective To explore the practicability and effectiveness of the short-form mini-nutritional asseassment(MNA-SF)for the evaluation of nutritional status of the elderly in nursing homes,and to provide evidence for the simple detection of malnutrition and timely intervention in the elderly. Methods MNA-SF and Controlling Nutritional Status(CONUT)were used to investigate the nutritional status of geriatric outpatients and wards from nursing homes,and the correlation and consistency of the two were analyzed. Results From the time of use,the COUNT method could not obtain the result on the same day,and the MNA-SF method only took 4 minutes. From the results,the proportion of normal nutrition measured by the COUNT method was 39.3%, and the proportion of mild, moderate and severe malnutrition was 30.4%,23.7% and 6.6%,respectively. The proportion of normal nutrition measured by the MNA-SF method was 36.6%,and the proportion of malnutrition was 63.4%. For comparison,the correlation coefficient between MNA-SF and COUNT(r =-0.863,P <0.01). Compared with COUNT,the sensitivity and specificity of MNA-SF were80.2% and 89.7%,respectively. Conclusion The incidence of malnutrition among the elderly in nursing homes is high.MNA-SF is a simple,easy and practical screening tool for nutritional status,which has a high consistency with the clinical evaluation methods related to disease.
