改良Mohler法修复单侧唇裂对称性改善的三维评价研究

摘　　要：目的:研究改良Mohler法修复单侧唇裂对恢复患者面部三维对称性的效果。方法:对广西医科大学附属口腔医院2017年1月至2018年12月收治的87例单侧唇裂患者实施改良Mohler法修复,术前、术后均应用面部结构光扫描采集面部三维形态数据,通过三维测量分析评价该方法对恢复面部三维对称性的效果。结果:与术前比较,术后患者各鼻唇部线距均得到明显改善,患侧鼻孔宽明显缩小,健侧唇高、患侧唇高及患侧唇长明显增加(均P<0.05);术前与术后健侧鼻孔宽、健侧唇长、健侧及患侧干唇宽度比较,差异均无统计学意义(均P>0.05)。与术前比较,术后患者鼻小柱基底-正中矢状面距离、人中切迹-正中矢状面距离两项距离指标均显著缩小,人中切迹-鼻小柱水平距离差显示人中切迹明显偏向患侧,双侧鼻孔宽度对称率、双侧唇高对称率、双侧唇长对称率明显改善(均P<0.05)。双侧干唇宽度对称率显示双侧干唇较好的保持了术前的宽度比例,差异无统计学意义(P>0.05)。结论:改良Mohler法修复单侧唇裂畸形,鼻唇部的三维形态对称性得到了明显的改善,可获得较为满意的外观,具有较强的临床实用性。Objective:To study the effect of modified Mohler method in repairing unilateral cleft lip on restoring facial three-dimensional symmetry.Methods:Eighty-seven patients with unilateral cleft lip in our hospital from January 2017 to December 2018 were selected.All patients were treated with modified Mohler method.Facial structure light scanning was performed to collect facial three-dimensional shape data pre-and postoperatively.The effect of this method on restoring the three-dimensional symmetry of the face was analyzed.Results:After surgery,the nasal and labial distance was significantly improved,the cleft side nostril width was narrowed,and the upper lip height and lip length of cleft side were increased(P<0.05).The unaffected side nostrilwidth,cleft side lip length and lip widthwere not notably changed(P>0.05).After surgery,the distance of nasal columella base to median sagittal plane and the distance of middle notch to median sagittal plane were significantly reduced.The horizontal distance of middle notch to nasal columella base showed that the middle notch was obviously deviated to the cleft side,and the symmetry rates of bilateral nostrils width,bilateral lips height and bilateral lips length were markedly improved(P<0.05).The symmetrical rate of the width of bilateral dry lips showed that the ratio of the width of bilateral dry lips before operation was well maintained(P>0.05).Conclusion:The modified Mohler method could repair the unilateral cleft lip deformity,and the three-dimensional morphological symmetry of the nasolabial part was obviously improved,and a satisfactory appearance was obtained.The method has good clinical applicability.
