摘 要:目的探索创伤情景模拟课程是否有利于住院医师临床能力的培养,提高住院医师创伤急救水平。方法实验组为中南大学湘雅三医院2016级第1年住院医师80人,分成10组,参加创伤急救情景模拟课程,对照组为采用传统方式进行教学的2015级第1年住院医师80人,课程结束后考核创伤急救流程及急救操作,统计比较两组心肺复苏、电除颤、止血包扎、骨折外固定、脊柱损伤搬运及团队合作等项目考核成绩的差异,并比较实验组学员培训前后考核成绩的差异,同时对实验组学员进行问卷调查。结果项目流程及操作的表现在心肺复苏和电除颤2个操作项目中差异无统计学意义,而止血包扎、骨折外固定和脊柱损伤搬运3个操作的考核成绩,实验组高于对照组,组间比较差异具有统计学意义(P<0.05);团队合作成绩,实验组高于对照组,组间比较差异具有统计学意义(P<0.05);实验组培训后成绩明显高于培训前,差异具有统计学意义(P<0.05)。结论在第1年住院医师中开展创伤急救情景模拟课程,与传统技能训练课程相比更有利于巩固学员院校教育的知识和技能,尤其对提高学员的团队合作精神有积极的意义,同时能提高住院医师的创伤急救水平。Objective This study aimed to explore whether the trauma scenario simulation courses are conductive to cultivate clinical competence and improve the abilities of dealing with trauma emergency for the residents.Methods The first-year resident in 2016 with a total of 80 in experimental group participated scenario simulation courses,and they were divided into 10 subgroups,and the control group with a total of 80 who were first-year residents in 2005 participated the traditional simulation courses.The results between experimental group and control group in cardiopulmonary resuscitation (CPR),defibrillation,hemostasis bandage,fracture external fixation,spinal injury handling and team-work performance were analyzed,performance before and after training were compared,and the feedbacks were surveyed in the form of questionnaires.Results The results of the experimental group after training were higher than those of control group in hemostatic bandage,fracture external fixation and spinal injury handling with significant differences(P< 0.05), whereas there were no significant differences in CPR and defibrillation between two groups.The results of the experimental group after training were higher than those of control group in team-work performance with significant differences(P < 0.05).The results of the experimental group after training were significantly higher than those before training with significant differences(P < 0.05).Conclusion Comparing to the traditional training courses,scenario simulation courses were more effective for residents in the first year to establish the knowledge framework and skills in college education,especially in teamwork project with incomparable advantages,furthermore,it can improve the levels of handing trauma emergency of residents.
