摘 要:住院医师规范化培训(住培)已在全国范围普遍开展,住院医师培训管理经过多年实践、探索、检验、反馈与改进,已逐步形成体系、步入正轨,专科医师规范化培训(专培)工作正崭露头角、方兴未艾。如何做好专科医师和住院医师培训的衔接工作,保证专科医师培训有序、稳步推进,是实现"住专一体化"管理的首要任务。本文试从培养目标、培训内容、培训阶段、教学方法、过程管理等方面阐述住院医师和专科医师培训实现一体化管理的基础、可行性和必要性,探讨如何达到培养目标的承接性、培训内容的递进性、教学方法的统一与个体化,如何在管理制度、过程监督中遵循统一原则,如何达到评估体系和而不同,如何共享优化信息化管理平台,探讨执业和学位双重培养机制的有机统一,为专培工作的全面推行提供思路与策略。Standardized residency training has been carried out widely and uniformly in China.With explorations,verification,feedback, improvement for years,a mature management system has been developed step by step.However,standardized training for specialists has just began.It's primary and critical for the integrated management of residency and specialist education to cohere specialist standardized training with standardized residency training,to ensure the march of specialist standardized training orderly and steadily.Herein,we summarized the program goals,training contents,rotation stages,teaching techniques,process supervisions and evaluations to explore the basis,feasibility and necessity of the integrated management of residency and specialist education.We demonstrated the methods of achieving the continuity in program goals,progressive training contents,the unity and individuality in teaching approaches,how to comply with the unifying principles in management regimes and process supervisions,how to achieve the harmony and diversity in evaluation systems,and how to share the optimized information management platforms.Furthermore,we discussed the mutual recognitions of registration,clinical practice and academic certifications in both training programs.In conclusion,this study might be able to provide an available strategy for implementation of specialist training programs.
