儿童失神癫痫的认知与脑电图的关系

摘　　要：目的分析儿童失神癫痫的脑电图背景活动及其与事件相关电位(event-related potential,ERP)P300的关系,为临床评估失神癫痫儿童的认知功能障碍及早期干预治疗提供客观依据。方法选取失神癫痫儿童56例(观察组),与其性别、年龄、文化程度相匹配的健康志愿儿童55例(对照组),收集其视频脑电图(video-electroencephalogram,VEEG)及ERP的P300潜伏期和波幅并进行回顾性分析。结果治疗前2组P300潜伏期及波幅差异均有统计学意义(P<0.05),治疗后2组P300潜伏期差异无统计学意义(P>0.05)。观察组治疗前后脑电图背景活动差异有统计学意义(P<0.05),P300潜伏期差异有统计学意义(P<0.05)。观察组脑电图背景活动与P300潜伏期呈负相关(P<0.05),即背景活动越慢,P300潜伏期越长。结论部分典型失神癫痫儿童存在认知受损,不典型失神儿童全部存在认知受损,且VEEG与P300存在明显相关性,两者结合可作为监测治疗效果的客观指标。Objective To analyze the background activity of electroencephalography(EEG)in childhood absence epilepsy and its relationship with event-related potential(ERP)P300,and to provide an objective basis for clinical evaluation of cognitive dysfunction and early intervention in childhood absence epilepsy.Methods A total of 56 children(observation group)with absence of epilepsy were enrolled.And 55 healthy volunteers(control group)matched with gender,age and education level were collected.The P300 latency and amplitude of video-electroencephalogram(VEEG)and ERP were collected and retrospectively analyzed.Results There was a statistically significant difference in the latency and amplitude of P300 between two groups before treatment(P<0.05).There was no significant difference in the latency of P300 between two groups after treatment(P>0.05).There were significant differences in the background activity of EEG between the observation group before and after treatment(P<0.05),and the difference of P300 latency was statistically significant(P<0.05).The background activity of EEG in the observation group was negatively correlated with the latency of P300(P<0.05),that is,the slower the background activity,the longer the P300 incubation period.Conclusion Some children with typical absence epilepsy have cognitive impairment,all atypical absence children have cognitive impairment,and VEEG has a significant correlation with P300,the combination of the two can be used as an objective indicator to monitor the therapeutic effect.
