摘　　要：目的研究非小细胞肺癌细胞EGFR表达、肿瘤类型与胸腔灌洗液肿瘤细胞存在的关系。方法通过留取非小细胞肺癌患者术中及术后胸腔灌洗液进行细胞学检测,同时将术后肺部肿瘤进行免疫组化分析,检测其CK、EGFR、P63表达情况,系统分析胸腔灌洗液阳性与肿瘤分子表达及肿瘤类型的关系。结果阳性组患者肿瘤大小位于3~7 cm发生率、纵膈淋巴结阳性发生率以及EGFR表达发生率高于阴性组,差异具有统计学意义(P<0.05);阳性组患者肿瘤小于3 cm以及大于7 cm,发生率低于阴性组,差异无统计学意义(P>0.05)。两组患者肿瘤位置、肿瘤类型、手术方式、手术范围以及CK、P53表达差异无统计学意义(P>0.05)。结论肿瘤的大小以及EGFR的高表达,与PLC阳性关系密切,可能是早期胸膜腔微转移的预测指标。Objective To study the relationship between molecular expression of non-small cell lung cancer cells EGFR,type of tumor and the existence of pleural lavage fluid tumor cells.Methods Cytological examination of thoracic lavage fluid in patients with non-small cell lung cancer during and after operation were performed,and the lung tumors were used to immunohistochemical analysis.The expressions of CK,CK7,CK19,TTF-1,EGFR,Ki-67 and P63 were detected,and the relationship between the positive expression of pleural lavage fluid,the expression of tumor molecules and type of tumor were systematically analyzed.Results The incidence of tumour size 3~7 cm,mediastinal lymph node positive rate and EGFR expression in the positive group were higher than those in the negative group.The difference was statistically significant(P<0.05).The incidence of tumors in the positive group with less than 3 cm and more than 7 cm was lower than that in the negative group.There was no statistically significant difference between the two groups(P>0.05).There was no statistically significant difference between the two groups in the tumor location,type,operation method,scope of operation and expression of CK and P53(P>0.05).Conclusion The size of tumors and the high expression of EGFR are closely related to the positive expression of PLC,which may be a predictor of early pleural micrometastasis.
