脑面血管瘤病16例视频脑电图与影像学分析

摘　　要：目的探讨脑面血管瘤病的视频脑电图(VEEG)以及影像学(MRI)表现,为临床早期诊断及监测预后提供依据。方法收集河北省儿童医院神经内科门诊及病房诊治的16例脑面血管瘤病患者的病例资料及检查结果,总结其VEEG与影像学表现。结果16例患儿在VEEG监测过程中监测到临床发作有6例,其中表现为局灶性发作者5例,痉挛发作者1例;发作间期放电形式有两种,一种为高度失律2例,一种为一侧半球棘波、棘慢波发放。影像学检查多在病变侧出现异常表现,脑萎缩或脑室变小5例,局部静脉影增多增粗5例,动脉远端分支增多增大1例,动脉稀疏减少1例,局部多发异常信号6例。同一患儿有一种或多种表现形式并存。结论影像学检查及VEEG是诊断脑面血管瘤病的主要手段之一,两者的异常表现大多出现在病变侧,VEEG在监测预后及可能并发癫痫的风险中作用重大。Objective To investigate the perfom of video-electroencephalogram(VEEG)and imaging of encephalofacial angiomatosis,andprovide basis for early diagnosis and prognostic evaluation.Methods The case data and examination results of 16 patients with encephalofacial angiomatosis diagnosed and treated in the outpatientdepartment and ward of the Children’s Hospital of Hebei were collected.Their VEEG and imagingcharacteristics were retrospectively analyzed and summarized,as well as the relevant between them.Results In the course of VEEG monitoring,6 of the 16 children were observed with clinical seizures,5 with focal seizure and 1 with spasm.There are two types ofinterparoxysmal discharge,one is hypsarrhythmia,the other is spike wave or spikeslow wave in onehemisphere.Imaging examination showed most abnormal manifestations appeared in the lesionside,including 5 cases of brain atrophy orventricle shrinkage,5 cases of increased and thickened localvenous shadow.1 case of increased distal arterialbranch,1 case of decreased arterial sparsityand 6 cases of local multiple abnormalsignals.One child has one or more abnomal manifestations at the same time.Conclusion maging examination and VEEG are one of the main means for diagnosis of encephalofacial angiomatosis,most of the abnormal manifestations occur in the lesion side.VEEG plays a significant rolein monitoring the prognosis and the risk of epilepsy.
