摘　　要：目的:观察颏下岛状瓣在修复口腔癌术后组织缺损中的应用价值。方法:收集2015年9月~2018年9月在我科收治的口腔癌患者行颏下岛状瓣修复组织缺损19例,记录病变部位、病理分型、临床分期等情况,设计颏下岛状瓣修复组织缺损,评估皮瓣成活、神经损伤、外形与功能恢复、癌肿复发和淋巴结转移等情况。结果:19例颏下岛状瓣均移植成功,其中2例血管危象,1例远端坏死,2例暂时性面瘫,所有受区与供区外观、功能满意,1例病灶复发,2例同侧淋巴结转移,1例远处转移。结论:颏下岛状瓣血供可靠,制备简单,应用灵活,成活率高,且供区与受区色泽、质地匹配,功能恢复满意,适宜基础疾病患者,值得临床推广应用。Objective:To discuss the clinical application and effects of submental artery island flap on repairing oral defects after cancer ablation.Methods:Nineteen patient,whose defects were repaired with submental artery island flap in oral regions after cancer ablation,the lesion site,pathological classification and clinical stage were recored simultaneously.The submental island flap was played to restore the tissue defect.The survival rate of flap,nerve injury,the recovery of shape and function,cancer recurrence and lymph node metastasis were evaluated respectively.Results:Nineteen cases of submental artery island flap were successfully transplanted,including two vascular crisis,one distal necrosis,and two temporary facial paralysis.The appearance and function of receiving area,and supplying area are satisfied.One case had primary recurrence,two cases had lymphaticmetastasis on the same side and one case had distant metastasis.Conclusion:There are some advantage on submental artery island flap such as reliable blood supply,simple preparation,high survival rate,good matching of color and texture on donor and recipient area,accompany with satisfactory functional recovery,suitable for the patients with underlying diseases.There is a good prospect in clinical practice.
