家族性甲状腺癌组织中BRCA1表达水平及其与临床病理特征分析

摘　　要：目的探讨乳腺癌易感基因1(BRCA1)在家族性甲状腺癌组织中的表达水平及其特点。方法回顾性分析本院42例正常甲状腺组织标本(对照组)、40例散发性甲状腺癌组织标本(散发性组)和45例经手术切除的家族性甲状腺癌组织标本(家族性组)的资料。采用免疫组织化学荧光染色法对各组标本BRCA1蛋白表达阳性率进行检测,并用实时荧光定量PCR法对各组标本BRCA1 mRNA相对表达量进行检测,并比较各组BRCA1蛋白和mRNA表达水平的差异。分析BRCA1蛋白表达水平与家族性甲状腺癌患者临床病理参数的相关性。结果与对照组(92.86%)、散发性组(87.50%)相比,家族性组BRCA1蛋白表达阳性率(62.22%)均下降(P均<0.05);而散发性组BRCA1蛋白表达阳性率与对照组比较差异无统计学意义(P>0.05)。家族性组BRCA1 mRNA相对表达量为(0.22±0.03),均低于对照组(1.00±0.10)和散发性组(0.98±0.05)(P均<0.05);而散发性组BRCA1 mRNA相对表达量与对照组比较差异无统计学意义(P>0.05)。家族性甲状腺癌患者年龄、双侧癌、被膜侵袭、淋巴结转移、同一家族中超过2例患甲状腺癌等临床病理参数BRCA1蛋白表达阳性率升高(P<0.05),而在性别、肿瘤直径、是否伴良性结节中差异无统计学意义(P>0.05)。结论家族性甲状腺癌组织中BRCA1蛋白和mRNA具有低表达的特点,并且BRCA1蛋白表达阳性率与患者年龄、双侧癌、被膜侵袭、淋巴结转移、同一家族中超过2例患甲状腺癌的临床病理参数密切相关,BRCA1可能是判断家族性甲状腺癌生物学行为的一种新型标志物。Objective To investigate the expression level and characteristics of breast cancer susceptibility gene 1(BRCA1)in familial thyroid carcinoma.Methods Total 42 normal thyroid tissue specimens(control group),40 sporadic thyroid cancer specimens(sporadic group)and 45 family thyroid cancer specimens(familial group)were retrospectively analyzed.Immunohistochemical fluorescence staining was used to detect the positive rate of BRCA1 protein expression in each group,and real-time fluorescence quantitative PCR was used to detect the relative expression of BRCA1 m RNA in each group,and the difference of BRCA1 protein and m RNA expression levels in each group was compared.The correlation between BRCA1 protein expression and clinicopathological parameters of familial thyroid cancer was analyzed by statistics.Results Compared with thecontrolgroup(92.86%)and sporadicgroup(87.50%),thepositiverateof BRCA1 protein expression in familial group(62.22%)was significantly decreased(P all<0.05),while the positive rate of BRCA1 protein expression in sporadic group was not significantly different from that of the control group(P>0.05).The relative expression of BRCA1 in familial group was(0.22±0.03),which was significantly lower than that of the control group(1.00±0.10)and sporadic group(0.98±0.05)(P all<0.05),while the relative expression of BRCA1 in sporadic group was not significantly different from that in control group(P>0.05).The positive rate of BRCA1 protein expression was closely correlated with age,bilateral cancer,capsular invasion,lymph node metastasis,thyroid cancer in more than 2 cases of the same family(P<0.05),but not with sex,tumor diameter and whether benign nodules were present(P>0.05).Conclusion BRCA1 protein and its m RNA in familial thyroid cancer are characterized by low expression.The positive rate of BRCA1 protein expression is closely related to age,bilateral cancer,capsular invasion,lymph node metastasis and clinicopathological parameters of more than 2 cases of thyroid cancer in the same family.BRCA1 may be
