摘　　要：目的研究p57^KIP2、p53、C-erbB-2蛋白在子宫内膜样癌(Endometrial adenocarcinoma,EA)中的表达情况,探讨三者与EA发生、发展的关系。方法运用Envision法检测100例EA组织中p57^KIP2、p53、C-erbB-2蛋白的表达情况(增生期、分泌期各30例为对照)及与临床病理特征的相关性。结果①p57^KIP2、p53与C-erbB-2蛋白在不同子宫内膜组织中的表达:p57^KIP2蛋白在分泌期中表达最高,在增生期中表达最低,总体比较,差异有统计学意义(P<0.05);组间比较,差异均有统计学意义(P≤0.017)。p53、C-erbB-2蛋白均在EA组织中表达最高,在分泌期中表达最低;p53蛋白在分泌期中不表达,总体比较,差异均有统计学意义(P<0.05);组间比较,两种蛋白均表现为在EA与增生期、EA与分泌期表达之间比较,差异有统计学意义(P≤0.017)。②p57^KIP2、p53与C-erbB-2蛋白在EA组织中阳性表达与临床病理特征的关系:在EA组织中,p57^KIP2蛋白在组织学Ⅰ级中表达最高,在Ⅲ级中表达最低,但仅在Ⅰ级和Ⅲ级、Ⅱ级和Ⅲ级中比较有统计学意义(P≤0.017);p53、C-erbB-2蛋白则在组织学Ⅲ级中表达最高,在Ⅰ级中表达最低,但仅C-erbB-2蛋白在Ⅰ级和Ⅱ级、Ⅰ级和Ⅲ级中表达有意义(P≤0.017)。3种蛋白的阳性表达均与肌层浸润深度有关,但仅p57^KIP2、C-erbB-2与手术分期有关(P<0.05)。三者均与淋巴结转移、年龄无关(P>0.05)。③p57^KIP2、p53与C-erbB-2蛋白表达之间的关系:仅p53和C-erbB-2蛋白表达之间有相关性(呈正相关)(P<0.05)。结论在EA组织中,p57^KIP2蛋白的低表达,p53、C-erbB-2蛋白的高表达提示EA患者有较高的组织学分级、手术分期、深肌层浸润,联合检测EA患者癌组织中p57^KIP2、p53与C-erbB-2蛋白的表达情况,对评估患者的预后有较为积极的指导意义。Objective To investigate the expression of p57^KIP2,p53 and C-erbB-2 proteins in endometrial adenocarcinoma(EA),and to explore the relationship between their expression and the occurrence and development of EA.Methods The expression of p57^KIP2,p53 and C-erbB-2 protein in 100 EA tissues was detected by Envision method(30 cases in the proliferative phase and secretory phase),and their correlation with clinicopathological features was analyzed.Results(1)The expression of p57^KIP2,p53 and C-erbB-2 protein in different endometrial tissues:p57^KIP2 protein was the highest in the secretory phase and lowest in the proliferative phase.The overall difference was statistically significant(P<0.05).There were significant differences between the groups(P≤0.017).The expression of p53 and C-erbB-2 protein was the highest in EA tissues and the lowest in secretory phase.p53 protein was not expressed in the secretory phase.The difference was statistically significant(P<0.05).The protein expression was compared between EA and proliferative phase,EA and secretory phase,and the difference was statistically significant(P≤0.017).(2)The relationship between the positive expression of p57^KIP2,p53 and C-erbB-2 protein in EA tissues and clinicopathological features:in EA tissues,p57^KIP2 protein was the highest in histological grade Ⅰ and lowest in grade Ⅲ,but only In grades Ⅰ and Ⅲ,grades Ⅱ and Ⅲ were statistically significant(P≤0.017);p53 and C-erbB-2 proteins were highest in histological grade Ⅲ and lowest in grade Ⅰ,but only C-erbB-2 protein expression was meaningful in grades Ⅰ and Ⅱ,grades Ⅰ and Ⅲ(P≤0.017).The positive expression of all three proteins was related to the depth of myometrial invasion,but only p57^KIP2 and C-erbB-2 were associated with surgical staging(P<0.05).All three were not related to lymph node metastasis and age(P>0.05).(3)Relationship between p57^KIP2,p53 and C-erbB-2 protein expression:There was a correlation(positive correlation)between p53 and C-erbB-2 protein expression(P<0.0
