河北省6所三甲医院急诊科护士道德困境现状调查及其影响因素分析

摘　　要：目的:探讨三甲医院急诊科护士道德困境现状及其影响因素。方法:选取某市6所三甲医院2018年1月至2019年1月415名急诊科护士作为研究对象,调查其基本情况,采用道德困境量表调查道德困境。结果:三甲医院急诊科护士的道德困境得分为71.28±10.93。单因素分析显示,三甲医院急诊科护士道德困境得分在年龄、学历、职称、护龄、是否结婚、工资水平、聘用方式和有无子女的比较差异具有统计学意义(P<0.05)。多元线性回归分析显示,学历、职称、护龄、工资水平和是否在编均是道德困境的显著影响因素(P<0.01)。结论:三甲医院急诊科护士道德困境相对较低,急诊科护理管理者应积极创造机会,为低学历及刚入职护士提供更多在职教育和入职培训,提升其学历水平、职业素养和技能理论知识;建立科学的绩效考核和职称晋升体系,缩小在编与非在编护士间待遇水平,实施同工同酬;建立工作上互相协作及学习和生活上互相帮助及关心的团结协作的集体氛围,增加护士对集体归属感。Objectives: To explore the current situation of moral evasion tendency and its influencing factors in emergency department in top tertiary hospital. Methods: 415 emergency nurses were selected as the research objects by convenience sampling from 6 top tertiary hospitals in a city during January 2017 to January 2018. The basic situation of nurses was investigated, and the tendency of nurses' moral evasion was investigated by Moral Evasion Scale. Results: The score of moral evasion tendency of nurses in emergency department of the major hospital was 72.18 ±12.93. Univariate analysis showed that there were significant differences in the scores of nurses' moral evasion tendency among age, educational background, professional title, nursing age, marriage, wage level, employment mode and children (P<0.05).Multivariate linear regression analysis showed that educational background, professional title, nursing age, wage level and whether it was edited or not were significant influencing factors of moral evasion tendency(P<0.01). Conclusions: Nurses in emergency departments of the major hospital have a relatively low tendency of moral evasion, and their educational background, professional title, nursing age, salary level and whether they are organized or not are the influencing factors of their moral evasion tendency.
