健脾消积汤联合化疗对老年晚期胃癌患者生活质量及免疫功能的影响

摘　　要：目的观察健脾消积汤联合化疗对老年晚期胃癌患者生活质量及免疫功能的影响。方法通过随机数字表法将73名患者随机分成治疗组(37例)和对照组(36例)。对照组FOLFOX4方案化疗,治疗组在对照组的基础上于化疗前2 d加用健脾消积汤治疗。1个周期为14 d,使用4个周期后进行疗效评价。观察对比2组患者之间治疗前后生活质量积分、免疫功能、中医证候积分、KPS评分及不良反应变化情况。结果治疗组治疗后生活质量积分、中医证候积分和KPS评分均有改善,与治疗组治疗前的评分和对照组治疗后评分比较,差异显著,有统计学意义(P<0.05)。对照组治疗后中医证候积分相应提高而生活质量积分、KPS评分等降低,与对照组治疗前比较,差异明显,有统计学意义(P<0.05)。化疗4个周期后,治疗组CD^3+、CD^4+、CD^8+、CD^4+/CD^8+水平较治疗前提高(P<0.05),对照组CD^4+、CD^4+/CD^8+水平较治疗前明显降低(P<0.05)。2组间比较,治疗组治疗后CD^4+、CD^4+/CD^8+水平较对照组治疗后高(P<0.05)。治疗组不良反应发生率低于对照组(P<0.05)。结论健脾消积汤联合化疗治疗老年晚期胃癌在改善临床症状、提高机体免疫功能、提高生活质量、减轻不良反应及延长生存期等多方面具有明显优势。Objective To observe the effect of Jianpi Xiaojie decoction combined with chemotherapy on the quality of life and immune function of senile patients with advanced gastric cancer.Methods 62 cases of patients were randomly divided into treatment group(32 cases)and control group(30 cases).The control group was treatmented by chemotherapy of FOLFOX4,and the treatment group was given Jianpi Xiaojie decoction on 2 days before chemotherapy.2 weeks was a course of treatment in two groups.The changes of TCM syndrome integral,KPS score,life Quality,immune function and adverse reaction after four courses of continuous treatment were observed.Results The TCM syndrome integral,life Quality and KPS score were achieved after treatment in the treatment group were more achieve than those in the control group(P<0.05).The TCM syndrome integral was achieved,life Quality and KPS score were debased in control group after theatment(P<0.05).After 4 periods of chemotherapy,CD^4+ and CD^4+ /CD^8+ levels were debased apparently as compared with those before treatment in the control group(P<0.05).The levels of CD^3+,CD^4+ ,CD^8+ and CD^4+ /CD^8+ were all increased as compared with those before treatment in the treatment group(P<0.05).In comparison of the two groups,the levels of CD^4+ and CD^4+ /CD^8+ in the treatment group were higher than those in the control group after treatment(P<0.05).The incidence of the adverse reactions in the treatment group was lower than that in the control group(P<0.05).Conclusion Jianpi Xiaojie decoction combined with chemotherapy can improve immune function,get obviously symptoms improvement,reduce incidence of the adverse reactions,has better quality,and prolong the survival time.
