摘  要:目的:研究枣梨、山楂口服液对高脂血症胰腺炎(hvperlipidemia pancreatitis,HAP)患者脂代谢及脂蛋白酯酶(lipoproteinesterase,LPL)基因表达的影响。方法:选取2017年12月至2018年12月就诊于我院肝胆胰外科的60例HAP患者为研究对象,分为对照组与实验组各30例。对照组予以常规的禁食、胃肠减压、抑制胰酶分泌、抑酸、抗感染等治疗;实验组在对照组基础上加用精制枣梨、山楂口服液进行干预,持续2周后测定血清中总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)和低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)等生化指标,以此评定枣梨、山楂对HAP患者降血脂的作用;用实时荧光定量RT-PCR法测定肝组织中LPL的mRNA表达水平,Western blot测定LPL蛋白含量,从分子水平研究枣梨、山楂口服液降血脂的机制。结果:两组患者治疗前后血清TC、TG、HDL-C、LDL-C等指标比较差异均具有统计学意义(P<0.05);与对照组相比,实验组患者LPLmRNA及蛋白在肝脏组织中的表达量明显升高,差异具有统计学意义(t=3.791,P=0.006)。结论:枣梨、山楂口服液能有效调控HAP患者LPL基因的表达,进而缓解胰腺炎病情进展。Objective:To investigate the effect of oral liquid of Rosa roxburghii Tratt and Crataegus pinnatifida on the lipid metabolism and lipoprotein lipase(LPL)gene expression in patients with hyperlipidemic acute pancreatitis(HAP).Methods:A total of 60 patients with HAP who attended Department of Hepatobiliary and Pancreatic Surgery in our hospital from December 2017 to December 2018 were enrolled and divided into control group and experimental group,with 30 patients in each group.The patients in the control group received routine treatments such as fasting,gastrointestinal decompression,inhibition of pancreatic enzyme secretion,anti-acid therapy,and anti-infective therapy,and those in the experimental group were given refined oral liquid of Rosa roxburghii Tratt and Crataegus pinnatifida in addition to the treatment in the control group.After 2 weeks of treatment,serum levels of total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C)were measured to evaluate the antihyperlipidemic effect of Rosa roxburghii Tratt and Crataegus pinnatifida in patients with HAP.Quantitative real-time PCR was used to measure the mRNA expression of LPL in liver tissue,and Western blot was used to measure the protein expression of LPL,so as to explore the antihyperlipidemic mechanism of the oral liquid of Rosa roxburghii Tratt and Crataegus pinnatifida.Results:Both groups had significant changes in serum TC,TG,HDL-C,and LDL-C after treatment(P<0.05).Compared with the control group,the experimental group had significant increases in the mRNA and protein expression of LPL in liver tissue(t=3.791,P=0.006).Conclusion:Oral liquid of Rosa roxburghii Tratt and Crataegus pinnatifida can effectively regulate the expression of LPL gene in patients with HAP and thus delay the progression of pancreatitis.
