成人注意缺陷多动障碍诊断访谈中文修订版的效度和信度

摘　　要：目的:引进成人注意缺陷多动障碍诊断访谈2.0中文修订版(DIVA2.0-CR),评价其效度和信度。方法:以上海交通大学医学院附属新华医院临床心理科门诊的疑似存在注意缺陷多动障碍诊断的成人患者35例及医院工作人员10人为研究对象,以临床诊断为金标准,评价DIVA2.0-CR效度。6名评定者分别独立对被试进行DIVA诊断,考察评定者间一致性。其中12例患者在DIVA访谈后7~14d,再次接受DIVA访谈,考察重测信度。结果:DIVA2.0-CR的评定者间一致性kappa值为0.83,重测信度为0.88。以临床诊断为金标准,对DIVA2.0-CR进行效度检验显示,诊断一致性的kappa值为0.76,诊断成人注意缺陷多动障碍的灵敏度为90%,诊断特异度为94%。结论:成人注意缺陷多动障碍诊断访谈中文版具备较高的效度和信度,可用于临床成人ADHD的诊断,具备较好的诊断灵敏度和特异度。Objective:To test the validity and reliability of the Chinese version of the Diagnostic Interview for ADHD in adults(DIVA).Methods:DIVA was translated into Chinese and administered to 35 patients in the outpatient clinic with the chief complaint being attention deficit related symptoms and 10 employees of Xinhua Hospital.The clinical diagnose by senior chief psychiatrist was the gold standard as to explore the validity of DIVA.Six interviewers were included in the inter-rater reliability training.Seven to 14 days after the first DIVA interview,12 participants were assessed with the second DIVA interview to evaluate the test-retest reliability.Results:The interrater reliability was 0.83 and the test-retest reliability was 0.88.Using clinical diagnose as the gold standard,the diagnostic consistency was 0.76.The validity test showed that the sensitivity was 90%for adult ADHD,and the specificity was 94%.Conclusion:The Chinese version of DIVA has good validity and reliability for Chinese adult ADHD diagnose.
