活血消瘤汤结合亮丙瑞林治疗子宫肌瘤疗效及对微血管密度的影响

摘　　要：目的评价活血消瘤汤结合亮丙瑞林治疗子宫肌瘤的临床疗效及对微血管密度的影响。方法选择130子宫肌瘤(气滞血瘀症)患者为研究对象,根据Excel随机表格法将患者分为研究组和对照组,每组65例。对照组患者采用醋酸亮丙瑞林微球治疗,研究组在采用醋酸亮丙瑞林微球治疗的基础上,口服活血消瘤汤治疗。两组均治疗3个月。比较两组治疗疗效,治疗前后临床症状积分,治疗前后平均肿瘤体积和子宫体积,治疗前后性激素水平,治疗前后子宫内膜COX-2、VEGF和微血管密度。结果研究结果显示,治疗组治疗总有效率为90.77%高于对照组治疗总有效率的76.92%(P<0.05);研究组治疗后中医症候积分低于对照组(P<0.05);研究组治疗后平均肿瘤体积和平均子宫体积小于对照组(P<0.05);研究组治疗后性激素水平E2、P、FSH和LH水平低于对照组(P<0.05);研究组治疗后子宫内膜COX-2、VEGF和微血管密度表达低于对照组(P<0.05)。结论活血消瘤汤治疗子宫肌瘤疗效确切,能够有效的缓解子宫肌瘤的临床症状,缩小子宫体积和肿瘤体积,降低雌、孕激素,降低子宫内膜COX-2、VEGF和微血管密度表达,和亮丙瑞林联合应用,标本兼职,临床治疗效果得以有效提升。Objective To evaluate the clinical efficacy of Huoxue Xiaoliu Decoction combined with leuprorelin in the treatment of uterine leiomyoma and its effect on microvessel density.Methods A total of 130 patients with uterine leiomyoma(Qi stagnation and blood stasis sydrnome)were selected as the research subjects.According to Excel random table method,the patients were divided into study group and control group,65 cases in each group.The control group was treated with leuprorelin acetate microspheres,while the study group was treated with Huoxue Xiaoliu Decoction on the basis of leuprorelin acetate microspheres.Both groups were treated for 3 months.The therapeutic effect,clinical symptom score,average tumor volume and uterine volume before and after treatment,sex hormone level before and after treatment,endometrial COX-2,vascular endothelial growth factor and microvessel density before and after treatment were compared between the two groups.Results The results showed that the total effective rate of study group was 90.77%,which was higher than that of control group(76.92%)(P<0.05).The score of TCM symptoms in study group was lower than that in control group(P<0.05).The average volume of tumors and uterus in study group was smaller than that in control group(P<0.05)and the levels of sex hormones E2,P,FSH and LH in study group were lower than those in the control group(P<0.05).The expressions of COX-2,vascular endothelial growth factor and microvessel density in the study group were lower than those in the control group(P<0.05).Conclusion Huoxue Xiaoliu Decoction is effective on treating uterine leiomyoma.It can effectively alleviate the clinical symptoms of uterine leiomyoma,reduce uterine and tumor volume,estrogen and progesterone,the expression of COX-2,vascular endometrial growth factor and microvessel density.Combined with leuprorelin,the clinical therapeutic effect can be effectively improved.
