摘  要:目的分析胃癌病人的K RAS蛋白表达及基因状态与临床病理特征的关系,为胃癌病人的个性化治疗提供指导。方法选取2016年1月至2017年12月南通市肿瘤医院314例胃癌病人,分别采用免疫组织化学法及荧光定量PCR的方法分析K RAS蛋白表达水平和基因突变状态。结果314例胃癌组织中K RAS蛋白表达与胃癌病人的年龄(P=0.6485)、性别(P=0.5858)、肿瘤大小(P=0.7611)、脉管侵犯(P=0.5524)、淋巴结转移均无明显关系(P=0.5064),与组织学分级密切相关(P=0.0195);K RAS基因突变类型包括G12D、G13D、G12A、G12S、G13S;不同性别(P=0.4566)、年龄(P=0.5242)、组织分化之间(P=0.7474)的K RAS突变率差异无统计学意义;K RAS基因突变率与蛋白表达检出率差异有统计学意义(P=0.0377)。结论K RAS的蛋白表达程度与肿瘤的分化程度有关,K RAS基因在不同性别、年龄、组织分化之间的突变率差异无统计学意义,且K RAS基因突变率与蛋白表达不一致。Objective To analyze the relationship between K RAS protein expression and gene status and clinicopathological fea tures in patients with gastric carcinoma,and to provide guidance for personalized treatment of gastric carcinoma patients.Methods A total of 314 patients with gastric cancer in Nantong Cancer Hospital from January 2016 to December 2017 were selected.The ex pression level of K RAS protein and the state of gene mutation were analyzed by immunohistochemical method and fluorescent quantitative PCR.Results The expression of K RAS protein in 314 cases of gastric carcinoma had no significant relationship with age(P=0.6485),sex(P=0.5858),tumor size(P=0.7611),vascular invasion(P=0.5524)and lymph node metastasis(P=0.5064),which was closely related to histological grade(P=0.0195).K RAS gene mutation types include G12D,G13D,G12A,G12C,G13S;K RAS mutation rates were not significantly different between genders(P=0.4566),age(P=0.5242),and tissue differentiation(P=0.7474).K RAS gene mutation rate and protein expression detection rate were statistically different(P=0.0377).Conclusion The protein expression of K RAS is related to the degree of tumor differentiation.There is no significant difference in the mutation rate between K RAS gene in different gender,age and tissue differentiation,and the K RAS gene mutation rate is inconsistent with protein expression.
