人乳头瘤病毒分型与Cervista检测在宫颈内皮病变中的诊断价值

摘　　要：目的比较人乳头瘤病毒(HPV)分型与Cervista检测两种方法在宫颈上皮内瘤变(CIN)中的诊断价值。方法选取2017年1月~2018年12月我院经新柏氏液基细胞学(TCT)检测并经病理活检确诊为鳞状上皮内低度瘤样病变(CIN1)106例和高度瘤样病变(CIN2+)210例患者作为研究对象,采集宫颈脱落细胞同时进行HPV分型和Cervista检测,比较两种检测的诊断效果。结果HPV分型和Cervista检测对14种高危型HPV的检测结果一致率为87.34%(276/316),KI值为0.6377。HPV分型检测在CIN1组中阳性率(51.88%)高于Cervista检测的33.96%,差异有统计学意义(P<0.05),但在CIN2+组中两种方法阳性率比较,差异无统计学意义(P>0.05)。结论两种检测方法一致性良好,在宫颈癌筛查中的诊断价值相当。HPV分型检测在CIN1中有较高的阳性率。Objective To compare the diagnostic value of two methods of human papillomavirus(HPV)typing and Cervista detection in cervical intraepithelial neoplasia(CIN).Methods A total of 106 women with CIN1 and 210 women with CIN2+diagnosed by new baishi liquid-based cytology(TCT)and confirmed by cervical biopsy in our hospital from January 2017 to December 2018 were enrolled in this study.All participants underwent HPV typing and Cervista testing simultaneously.Then the diagnostic effect of the two tests was compared.Results The overall coincidence rate of the two HPV testing methods was 87.34%(276/316),and the KI value was 0.6377.The positive rate of HPV typing in CIN1 group was 51.88%,which was higher than that of Cervista testing(33.96%),the difference was statistically significant(P<0.05),but there was no significant difference between the two methods in the CIN2+group(P>0.05).Conclusion The two methods are in good consistency and have equivalent diagnostic value in cervical cancer screening.HPV typing test has a higher positive rate in CIN1.
