某地区四种高危型HPV病毒基因启动子上游长控制区与E2区基因突变的分析

摘　　要：目的探讨萍乡地区HPV16、18、52、58四种高危型人乳头瘤病毒(human papillomavirus,HPV)的长控制区以及E2区突变与宫颈病变的关系。方法选取2013年7月至2018年5月入萍乡市妇幼保健院住院妇科经病理明确诊断的宫颈上皮内瘤变及宫颈癌患者,且单一感染HPV16、18、52、58四种型别,入组人数为176例。对四种高危型HPV病毒基因组的长控制区域(Long control region,LCR)和E2区域进行测序分析。结果萍乡地区HPV16型及52型的长控制区域和E2区含有大量突变位点,该突变可能与宫颈病变进展相关,且16、18、52、58型HPV病毒LCR区及E2区的突变比例在宫颈上皮内瘤变及宫颈癌患者中存在显著的差异。结论感染16、52、58型HPV的宫颈上皮内瘤变患者体内HPV病毒LCR区及E2区突变比例较在宫颈癌患者中的显著性增加,而18型HPV LCR区和E2区突变比例在宫颈癌患者更高,且保守型高,提示不同高危型别HPV通过LCR区或E2区突变导致不同程度的宫颈病变进展。Objective To investigatethe relation of the long control region and E2 region mutations of four high-risk human papillomavirus(HPV)with cervical lesions were studied in the Pingxiang area.Methods 176 patients infected by one kind of HPV16,18,52,58 types were selected,diagnosed as cervical intraepithelial neoplasia(CIN)and cervical cancer with pathological diagnosis from July 2013 to May 2018 in Pingxiang Maternal and Child Health Hospital.The long control region(LCR)and E2 region of these four high-risk HPV genomes were amplified and sequenced.Results The long control region and E2 region of HPV 16 and 52 types in Pingxiang area have de novo mutation,and the mutation ratios of LCR and E2 regions of HPV,16,18,52 and 58 types were different from cervical intraepithelial neoplasia and cervical cancer.Conclusion There are a large number of de novo mutations in the LCR and E2 regions of HPV 16,18,52,58 types associated with cervical lesions.The rate of mutations in the LCR and E2 regions of HPV 16,52 and 58 types in patients with cervical intraepithelial neoplasia were significantly higher than that in patients with cervical cancer,while the higher rate of mutations in LCR and E2 in HPV 18 type were higher in patients with cervical cancer.And it suggests that the mutation ratio of LCR and E2 region of high-risk type HPV may lead to different degrees of cervical lesions.
