摘 要:目的比较帕金森病(PD)患者与正常对照大脑感觉运动控制与调节功能区的时间动态功能网络连接(tdFNC)状态,分析PD患者各独立成分内连接强度与临床运动评分的相关性。资料与方法对30例PD患者及34例正常对照者进行静息态功能MRI扫描。通过独立成分分析法(ICA)软件将全脑分为33个独立成分(IC),比较双侧感觉运动网络、岛叶网络及小脑网络各IC之间的tdFNC,并经FDR多重比较校正,观察组间差异;分析PD患者各IC的连接强度与临床评分的相关性。结果PD患者与正常对照者tdFNC中state 2存在显著差异。双侧躯体感觉皮层IC 1与IC 13之间连接功能减低,差异有统计学意义(t=-3.02,P<0.05);岛叶网络IC 30与小脑网络IC 8功能连接减低(t=-2.78,P<0.05);同时state 2的平均驻留时间明显升高(t=2.28,P<0.05)。临床运动功能评分与左侧躯体感觉皮层(r=-0.734)、感觉运动皮层(r=-0.367)及小脑网络(r=-0.416)均呈负相关(P<0.05)。结论PD患者动态功能网络连接异常及脑网络独立成分与运动评分之间的相关性可为本病的运动功能异常等症状提供客观解释,并为进一步研究及诊断提供参考。Purpose To compare the time dynamic function network connection(tdFNC)status of sensory motor control and regulatory functional areas in patients with Parkinson’s disease(PD)and normal controls,and to analyze the correlation between the strength of each independent component of PD patients and clinical motor scores.Materials and Methods Thirty patients with PD and 34 normal controls were scanned for resting functional MRI.Through the independent component analysis(ICA)software,the whole brain was divided into 33 independent components(IC),and the tdFNC between the IC of bilateral sensorimotor network,insula network and cerebellum network was compared,and corrected by FDR multiple comparisons to observe the differences between groups;and the correlation between the connection strength of each IC in PD patients and clinical scores was also evaluated.Results There was significant difference in state 2 between PD patients and normal controls in tdFNC.The connection function between IC 1 and IC 13 in bilateral somatosensory cortex was statistically decreased(t=-3.02,P<0.05);the functional connection between IC 30 of insular network and IC 8 of cerebellar network was decreased(t=-2.78,P<0.05);meanwhile,the average residence time of state 2 increased significantly(t=2.28,P<0.05).The clinical motor function score was negatively correlated with the left somatosensory cortex(r=-0.734),sensorimotor cortex(r=-0.367)and cerebellar network(r=-0.416)(P<0.05).Conclusion The different changes of tdFNC in PD patients and the relationship between IC and clinical scores can provide an objective and reliable explanation and methods for the next further research and clinical diagnosis of the disease.
