血清PSA、F-PSA、HE4在前列腺癌中的表达及意义

摘　　要：目的探究血清前列腺特异抗原(PSA)、游离PSA(F-PSA)、人附睾蛋白4(HE4)在前列腺癌(PCa)中的表达及意义。方法回顾性分析140例PCa患者(PCa组)及84例前列腺良性增生(BPH)患者(BPH组)临床资料,并纳入同期健康体检男性62例作为健康对照组。比较3组血清PSA、F-PSA、HE4水平,使用受试者工作特征曲线(ROC曲线)评估血清PSA、F-PSA、HE4对PCa的诊断价值,并分析血清PSA、F-PSA、HE4与PCa患者Gleason前列腺腺癌组织学分级(Gleason评分)、淋巴结转移情况、临床分期等临床病理特征的关系。结果BPH组血清PSA、F-PSA、HE4水平高于健康对照组(P<0.05),PCa组血清PSA、F-PSA、HE4水平高于BPH组和健康对照组(P<0.05)。经ROC曲线分析,发现血清PSA、F-PSA、HE4对PCa均具有较高的诊断效能(P<0.05),其曲线下面积(AUC)分别为0.949(95%CI:0.926~0.971)、0.788(95%CI:0.736~0.840)、0.826(95%CI:0.777~0.847)。血清PSA、F-PSA、HE4在高龄、高Gleason评分、伴淋巴结转移、高临床分期的PCa患者中表达水平更高(P<0.05)。结论血清PSA、F-PSA、HE4不仅能作为诊断PCa的指标,且与PCa的发生发展密切相关,临床使用价值显著。Objective To explore the expressions and significance of serum prostate specific antigen(PSA),free PSA(F-PSA)and human epididymis protein 4(HE4)in prostate cancer(PCa).Methods Retrospective analysis was performed on the clinical data of 140 patients with PCa(PCa group)and 84 patients with benign prostatic hyperplasia(BPH)(BPH group),and another 62 men in the same period for physical examination were included as healthy control group.The levels of serum PSA,F-PSA and HE4 were compared among the 3 groups.The receiver operating characteristic curve(ROC curve)was used to evaluate the diagnostic value of serum PSA,F-PSA and HE4 on PCa,and the relationship between serum PSA,F-PSA and HE4 and clinicopathological features of Gleason prostate adenocarcinoma histological grading(Gleason score),lymph node metastasis and clinical staging of PCa patients was analyzed.Results The levels of serum PSA,F-PSA and HE4 in BPH group were higher than those of healthy control group(P<0.05),and the levels of serum PSA,F-PSA and HE4 in PCa group were higher than those of BPH group and healthy control group(P<0.05).According to ROC curve analysis,serum PSA,F-PSA and HE4 were found to have high diagnostic efficacy on PCa(P<0.05),and the values of area under the curve(AUC)were 0.949(95%CI:0.926~0.971),0.788(95%CI:0.736~0.840)and 0.826(95%CI:0.777~0.847).The expression levels of serum PSA,F-PSA and HE4 were higher among PCa patients with advanced age,high Gleason score,lymph node metastasis and high clinical stage(P<0.05).Conclusion Serum PSA,F-PSA and HE4 can not only be used as indicators for the diagnosis of PCa,but also are closely related to the occurrence and development of PCa,and they have significant clinical application value.
