摘 要:目的研究非小细胞肺癌中CD73与调节性T细胞的表达及其临床意义。方法采用免疫组化SP法检测CD73与Foxp3标记的调节性T细胞在70例非小细胞肺癌及10例癌旁正常肺组织的表达,分析其与临床病理其特征的相关性。Kaplan-Meier生存分析研究CD73及调节性T细胞与患者生存期的关系。单因素及Cox回归多因素分析非小细胞肺癌预后相关因素。结果 CD73与调节性T细胞在10例癌旁正常肺组织中均呈阴性表达。70例非小细胞肺癌组织中53例CD73呈阳性表达(73.7%),49例调节性T细胞呈阳性表达(70%)。CD73与调节性T细胞的表达与淋巴结转移、TNM分期及生存期相关(P<0.05)。CD73与调节性T细胞的表达有相关性(r=0.284,P<0.05)。在单因素分析中,患者预后与肿瘤大小、淋巴结转移、分化程度、TNM分期、CD73与调节性T细胞的表达相关(P<0.05)。Cox多元回归分析提示CD73与调节性T细胞是患者生存期的独立影响因素(P<0.01)。结论 CD73与调节性T细胞在非小细胞肺癌表达上调,有可能成为监测非小细胞肺癌病情及判断预后的有效指标。Objective To investigate the expression of CD73 and regulator T cells(Treg) in non-small cell lung cancer(NSCLC) and the clinical significance. Methods The expressions of CD73 and regulator T cells were detected by immunohistochemical staining in 70 cases of NSCLC patients and 10 cases of normal lung tissues. The correlations between expressions and clinicopathological features were statistically analyzed. Kaplan-Meier survival analysis was used to investigate the relationship between the prognosis and the expressions of CD73 and Treg. Univariate and multivariate analysis by Cox regression were used to study the prognostic factors of NSCLC. Results In 70 cases of NSCLC patients, the positive rate of CD73 and Treg were 73.7% and 70%, respectively. In 10 cases of normal lung tissues, there was no positive expression. Compared with normal lung tissues, the expressions of CD73 and Treg in NSCLC patients were significantly different(P<0.01). The expressions of CD73 and Treg had correlations with lymph node metastasis, TNM stage and survival time(P<0.05). By Spearman test, the significantly positive correlation was found between the expression of CD73 and Treg(P<0.05). Univariate analysis revealed that the prognosis was significantly correlated with tumor size, lymph node metastasis, pathological tissue grade, TNM stage and the expressions of CD73 and Treg(P<0.05). Multivariate analysis by Cox regression suggested that CD73 and Treg were independent influencing factors for patient’s prognosis(P<0.01). Conclusion The expressions of CD73 and Treg are up-regulated, which may be used as effective indicators to monitor the progression and prognosis of NSCLC.
