进行性肌营养不良症中医证型与肌酸激酶的相关性研究

摘　　要：目的:研究进行性肌营养不良症(progressive muscular dystrophy,PMD)的中医症型与血肌酸激酶(Creatine Kinase,CK)之间的相关性,寻找中西医结合治疗依据。方法:回顾性分析2008年1—12月收治的并经过肌肉病理明确诊断的PMD患者入组研究,进一步根据PMD亚型和中医证型分组,分析各组间血CK的关系。结果:中医证型提示DMD、FSHD和LGMD的肺热致痿症组较脾肾致痿症组血CK值均明显降低(P=0.013);DMD、FSHD和LGMD的脾胃虚弱致痿组较脾肾两虚致痿组CK值明显降低(P=0.037);PMD患者的发病年龄、病程与血CK线性回归分析呈负相关(P<0.05)。结论:PMD患者以虚证组最常见,血CK值变化趋势与中医痿症症型演变趋势存在正相关。Objective:To study the correlation between the traditional Chinese medicine symptom of progressive muscular dystrophy(PMD)and blood creatine kinase(CK),and to find the basis for the treatment of integrated traditional Chinese and western medicine.Methods:Retrospectively analyze the PMD patients admitted from January to December 2008 and diagnosed with muscular pathology.The group was further divided into groups based on PMD subtypes and TCM syndromes,and the relationship between blood CK between groups was analyzed.Results:The TCM syndrome type indicated that the DMD,FSHD and LGMD pulmonary heat-induced atrophy group had significantly lower blood CK values than the spleen and kidney-induced atrophy group(P=0.013);DMD,FSHD and LGMD had weak spleen and stomach-induced atrophy group than the spleen The CK value of kidney-deficiency and impotence group was significantly decreased(P=0.037);the age of onset and course of disease in PMD patients were negatively correlated with linear regression analysis of blood CK(P<0.05).Conclusion:PMD patients are most common in the deficiency syndrome group.The change trend of blood CK value is positively correlated with the evolution trend of Chinese medical atrophy.
