摘  要:目的探讨以案例为导向的客观结构化临床考试(objective structured clinical examination,OSCE)在本科实习生临床综合能力评价中的应用成效,为临床本科实习生考核提供一种新方法。方法对2018年7月—2019年4月在本院实习的本科实习生287名,实习9个月后采用以病例为导向的OSCE评价实习生临床综合能力,考核采用6个站点(分别为护理评估、护理诊断、体格检查、专科护理操作、基础护理操作、护患沟通技巧),实习生考核后计算得分率情况及实习生与老师对本次考核满意度反馈情况。结果287名实习生考试成绩为70.03~92.06分,平均(81.57±3.36)分。6个站点平均得分率为81.57%,其中得分率较高的为第5站点“基础护理操作”与第4站点“专科护理操作”,得分率分别为91.60%与90.70%,得分率较低的为第1站点“护理评估”与第6站点“护患沟通技巧”,得分率分别为70.47%与71.40%。实习生与老师对本次考试满意或赞同情况分别为66.55%~100.00%与74.63%~100.00%。结论以案例为导向的OSCE模式较传统考试方法更新颖、更公平合理,能锻炼实习生的临床思维,客观反应其临床综合能力,可为改进临床教学工作提供方向,值得推广应用。Objective To explore the effect of objective structured clinical examination(OSCE)in the evaluation over clinical comprehensive ability of nursing undergraduate interns.Methods A total of 287 nursing undergraduate students who interned in our hospital from July 2018 to April 2019 were evaluated by OSCE before the end of the internship.Six spots covering nursing assessment,nursing diagnosis,physical examinations,operation of special nursing,operation of basic nursing and nurse-patient communication skills were set up and in each one,the assessment took 8 minutes.The scores of the student were statistically treated.A questionnaire was used to investigate the satisfaction of students and teachers on the exam.Results The scores of the student at the exam were 70.03-92.06,averaged 81.57±3.36.The average score among the six spots was 81.57%,those at the spots of basic nursing and special nursing operations the highest at 91.60%and 90.70%,respectively and those at the two spots of case history taking and communications skills the lowest at 70.47%and 71.40%,respectively.The satisfaction rates of the students and teachers with the OSCE were 66.55%-100.00%and 74.62%-100.00%,respectively.Conclusions The OSCE is more novel,fairer and more reasonable than the traditional examination.It can objectively reflect the clinical comprehensive ability of nursing interns,and provide a direction for improving clinical teaching work,which is worth promoting.
