摘  要:目的分析Actin在乳腺浸润性导管癌中的表达及临床病理意义。方法采用免疫组化SP法检测117例乳腺浸润性导管癌组织中Actin的表达,分析Actin表达与其临床病理、分子分型的关系。结果①Actin在乳腺浸润性导管癌中的表达主要在间质细胞中,仅4例三阴性乳腺浸润性导管癌(TNBC)同时存在癌细胞和间质细胞表达;而Actin在正常乳腺及癌旁乳腺组织中的肌上皮细胞均有表达,乳腺导管上皮及间质细胞则无表达。②Actin在乳腺浸润性导管癌间质细胞中的高表达水平与肿瘤直径及淋巴结转移相关(P<0.05),与年龄、组织学分级及临床分期无关(P>0.05)。③Actin在乳腺浸润性导管癌Luminal A型、Luminal B型(Her2+)、Luminal B型(Her2-)、Her2受体(+)和三阴性(TNBC)型的间质细胞中阳性率分别为28.6%、22.4%、42.9%、59.1%和81.3%,Actin在TNBC中的表达水平明显高于Luminal A型、Luminal B型(Her2+)、Luminal B型(Her2-)的表达水平(P<0.05),Her2受体阳性中Actin的表达水平高于Luminal B型(Her2+)的表达水平(P<0.05),其他亚型间两两比较差异不显著(P>0.05)。结论Actin不仅在乳腺浸润性导管癌不同分子亚型中的表达存在差异,并且与肿瘤大小及淋巴结转移有关。检测Actin在乳腺浸润性导管癌中的表达,有助于为乳腺癌不同分子亚型的鉴别诊断及个体化靶向治疗(特别是TNBC型)提供新的研究方向。Actin在乳腺癌与正常乳腺及癌旁组织中的差异表达有利于乳腺良、恶性病变的鉴别诊断。Objective To investigate the expression of actin in breast invasive ductal carcinoma and its clinicopathological significance.Methods Immunohistochemical examination was performed in 117 cases of breast invasive ductal carcinoma,40 cases of paracancerous tissue and 20 cases of normal breast tissue,and the relation between actin expression and clinicopathological features,molecular subtyes was studied.Results The positive sites of actin expression in breast invasive ductal carcinoma were mainly in matrix cells;only four cases with triple-negative breast cancer(TNBC)were both in matrix cells and duct epithelial cells,and in paracancerous tissue and normal breast tissuse actin expression was noted in myoepithelial cells.Based on immunohistochemical staining,high positive expression rate of actin was associated with tumor diameter and lymph node metastasis(P<0.05).There was no correlation between actin expression and age,histological grade and clinical stages(P>0.05).The positive rate of actin expression in TNBC,human epidermal receptor 2 positive(Her2+),luminal A,luminal B(Her2+)and lminal B(Her2-)subtypes were 81.3%,59.1%,28.6%,22.4%,and 42.9%,respectively.The expression of actin was significantly increased in TNBC compared with that in luminal A,luminal B(Her2+)and luminal B(Her2-)subtypes,and the expression of actin was higher in Her2+than that in luminal B(Her2+)(P<0.05),and there was no differences between other subtypes(P>0.05).Conclusion The expression of actin in different molecular subtypes of breast invasive ductal carcinoma shows significant difference,and is also correlated with tumor diameter and lymph nodes.To study the expression of actin may provide an evaluation for targeted treatment(especially for TBNC subtype)and contributes to their diagnosis of different molecular subtypes.The different positive sites of actin expression in breast invasive ductal carcinoma,paracancerous tissue and normal breast tissuse may be helpful for their differential diagnosis.
