基于认知-行为的心理护理对青年脑卒中后抑郁的康复效果的影响

摘　　要：目的探讨基于认知-行为的心理护理对青年脑卒中后抑郁的康复效果的影响。方法选取本院2018年6月~2019年11月收治的114例青年脑卒中后抑郁患者为研究对象,按随机数字表法将研究对象随机分为观察组和对照组,每组各57例患者。对照组给予常规护理,观察组在此基础上开展基于认知-行为的心理护理,连续护理2周。观察比较两组患者治疗前后汉密尔顿抑郁量表(HAMD)评分、汉密尔顿焦虑量表(HAMA)评分、美国国立卫生研究院卒中量表(NIHSS)评分、简明健康状况调查表(SF-36)评分以及简明精神状态检查量表(MMSE)评分。结果两组患者治疗后HAMA评分、HAMD评分、NIHSS评分均显著降低,且观察组评分明显低于对照组(P<0.05);两组患者治疗后SF-36评分、MMSE评分均显著升高,且观察组评分明显高于对照组(P<0.05)。结论基于认知-行为的心理护理能够有效改善青年脑卒中后抑郁患者的焦虑、抑郁情绪,有助于改善神经及认知功能,提升其生活质量。Objective To study the effect of cognitive-behavior-based psychological nursing on the rehabilitation effect of post-stroke depression in young adults.Methods A total of 114 young patients with post-stroke depression in our hospital from June 2018 to November 2019 were selected as the research subjects and randomly divided into two groups.The control group(57 cases)was given routine nursing,and the observation group(57 cases)was given cognitive-behavioral psychological nursing.Hamiltondepression scale(HAMD)score,Hamilton anxiety scale(HAMA)score,National institutes of health stroke scale(NIHSS)score,the 36-item shot-form health status survey(SF-36)score,and Simple mental state examination scale(MMSE)between the two groups were compared before and after nursing.Results HAMA score,HAMD score and NIHSS score in the two groups significantly decreased after treatment,and the score in the observation group was significantly lower than that in the control group(P<0.05);sf-36 score and MMSE score in the two groups were significantly increased after treatment,and the score in the observation group was significantly higher than that in the control group(P<0.05).Conclusion Cognitive-behavior-based psychological nursing can effectively reduce the anxiety and depression of young post-stroke depression patients,and help to improve neurological and cognitive functions and quality of life.
