摘 要:目的研究临床医师及医院管理者对亚专科设立及专业细化的看法和由此引发的新的管理问题,探索在专业细化的大背景下医院管理者的应对策略。方法对上海某三级甲等医院的临床医师及医院管理者进行问卷调查,内容涵盖被调查者对专业细化必要性、专业细化对学科建设的影响、专业细化的益处进行分析,以及可能存在的影响等方面。结果临床医师及管理者均认为应进行专业细分,设立亚专业,只有进行专业细分,才能走在专业的最前沿,掌握最新的医疗知识和技术。但细分到何种程度,医务人员之间有一定的分歧,主要担心医师综合思维能力可能会受到影响。结论专业细化是大势所趋,但也会带来一定的弊端,管理者应通过多种方式进行干预,才能发挥专业细化的优势,提高医院核心竞争力。Objective To investigate views of clinicians and hospital managers on subspecialty establishment and to explore more effective coping strategies of hospital managers in the context of specialty refinement. Methods A questionnaire survey was conducted among clinicians and administrators in a tertiary hospital in Shanghai. The content of the questionnaire covered multiple aspects, including the necessity of subspecialty establishment, impacts of subspecialty on discipline construction, the potentially positive or passive impact on subspecialty establishment, and so on. Results Both clinicians and hospital managers showed their approval on subspecialty establishment, which they thought would be beneficial to remaining at the forefront of the discipline and acquiring cutting-edge knowledge and technologies. However, clinicians with different education degrees had divided opinions on how much specialization should be made in one specialty, which might be affected by doctors’ comprehensive thinking ability. Conclusion Professional refinement is trending accompanied with some disadvantages. Hospital managers should take various interfering measures, in order to improve the core competence of the hospital.
