改良个案病例讨论在研究生教育中的应用研究

摘　　要：目的探讨改良个案病例讨论(CbD)在临床型硕士研究生医学教育中的应用效果。方法选取2017年7月—2018年6月济宁医学院附属医院进入内科轮转学习3年的临床型硕士研究生(即住院医师)共40名,应用“济医附院版CbD评量表”对不同年级住院医师进行评量。结果一次CbD评量发现,第一年轮转的住院医师得分最低,所有住院医师经过半年的内科系统轮转后评量情况均较第一次有明显的进步,尤其是第一年轮转的住院医师进步最明显;第三年轮转的住院医师除了临床评估诊断能力有大幅度提高,其他方面进步缓慢。结论CbD是一种简单实用、使用成本低、信效度高的临床能力评估工具。Objective To explore the application of improved case-based discussion(CbD)in medical education for clinical postgraduates.Methods A total of 40 clinical postgraduate students(residents)of three years in the rotation of internal medicine system at the affiliated hospital of Jining Medical University from July 2017 to June 2018 were selected,and evaluated by the CbD scale made by our hospital.Results In one CbD evaluation,residents in the first year of rotation had the lowest scores.After half year of rotation in internal medicine system,evaluation of all re-sidents improved significantly compared with the first time,among whom residents in the first year of rotation made the largest progress.For residents in the third year of rotation,they impro-ved slowly except for the large improvement in clinical assessing and diagnostic capability.Con-clusion CbD is a simple and practical clinical ability evaluating tool with low cost and high reli-ability.
