摘 要:目的观察短期综合干预(EONPS)对单纯性肥胖儿童体循环微小RNA(miR)-142-5p及miR-133b表达变化的影响,探讨其与体格数据、生化指标变化的关系及意义。方法对12例8~10岁的单纯性肥胖儿童进行为期3周的综合干预,对比干预前后体格数据、生化指标及miRNA-142-5p、miRNA-133b的表达水平变化,并分析其相关性。结果经过3周EONPS,12例单纯性肥胖儿童未观察到明显不适及不良事件,其体质量、腰围、腰围/身高、BMI、空腹胰岛素、总胆固醇、甘油三酯、胰岛素抵抗指数水平下降,胰岛素敏感指数增大(P均<0.05),而血压、空腹血糖、HDL-C、非HDL-C(non-HDL-C)等指标无明显变化。其体循环miR-142-5p表达值上升(P<0.05),其变化值与腰围/身高、血压、BMI差值呈正相关,与甘油三酯、LDL-C差值呈负相关(P均<0.05);miR-133b表达值上升不明显(P>0.05),其与体格测量数据及生化指标的变化无明显相关性。结论EONPS能有效降低肥胖儿童体质量、改善其体成分,提高胰岛素敏感性及体循环中miR-142-5p的表达水平,miR-142-5p或可作为肥胖儿童脂代谢紊乱的分子标志物。Objective To evaluate the effect of short-term Healthy education + aerobic exercise + reasonable nutrition + correct psychology + medical supervision(EONPS) on the expression levels of circulating miR-142-5 p and miR-133 b in children with simple obesity,and to explore the correlation with the changes of morphometric and biochemical parameters.Methods Three-week ENOPS was carried out among 12 children with simple obesity,aged 8-10 years old.Morphometric data,biochemical parameters and the expression levels of circulating miR-142-5 P and miR-133 b were quantitatively measured pre-and post-EONPS interventions.Results After 3 weeks of EONPS interventions,no significant discomforts or adverse events were observed in 12 children.Weight,waist circumference,waist-to-height ratio,body mass index(BMI),triglyceride,total cholesterol,fasting insulin and HOMA-IR were significantly decreased,whereas insulin sensitivity index(ISI) was remarkably increased(all P < 0.05).However,blood pressure,fasting blood glucose,high-density lipoprotein cholesterol and non-high-density lipoprotein cholesterol did not significantly differ(all P > 0.05).The expression level of circulating miR-142-5 p was up-regulated,the change of it was positively correlated with waist-to-height ratio,blood pressure and BMI,whereas negatively correlated with triglyceride and low-density lipoprotein cholesterol.The expression level of circulating miR-133 b had no significant change,the change of it was not correlated with any morphometric data or biochemical parameters.Conclusions Short-term EONPS can significantly reduce the body weight,improve the body composition,enhance the insulin sensitivity and up-regulate the expression level of circulating miR-142-5 p.Hence,miR-142-5 p can serve as a molecular maker of lipid metabolic disorders in children with simple obesity.
