摘　　要：目的探究头针联合下肢机器人康复对脑梗死患者步态的影响。方法以2018年12月至2019年5月山西中医学院第三中医院康复科住院康复的脑梗死恢复期患者60例作为研究对象,采用随机数字表,将以上患者随机分为A、B、C组,每组20例。A组患者采取头针治疗,B组患者采取下肢机器人治疗,C组患者采取头针联合下肢机器人治疗。比较3组患者的步态、患侧关节角度、Fugl-Meyer评测法(FMA)评分、日常生活活动能力(ADL)评分、应激反应之间的差异。结果C组患者的步行速度显著高于其他2组;步长不对称性、单腿支撑时间不对称性以及应激反应各项指标显著低于其他2组,C组患者髋关节最大后伸角度、支撑相膝关节最大背伸角度、膝关节最大屈曲角度、FMA评分、ADL评分显著高于其他2组。结论头针联合下肢机器人康复能明显提高脑梗死患者的步行速度,从而改善患者的步态模式,提高患者的日常生活能力。Objective To investigate the effect of scalp acupuncture combined with lower limb rehabilitation robot during gait of patients with cerebral infarction.Methods A total of 60 recovering patients with cerebral infarction hospitalized in the Department of Rehabilitation,Third Hospital of Traditional Chinese Medicine,Shanxi College of Traditional Chinese Medicine between December 2018 and May 2019 were included as subjects,and according to random number table,randomized into groups A,B,and C(n=20 each).The patients in group A were treated with scalp acupuncture,those in group B with lower limb rehabilitation robot,and those in group C with scalp acupuncture combined with lower limb rehabilitation robot.The three groups were compared for the gait,joint angle on ipsilateral side,FMA score,ADL score and stress response.Results Compared with the other two groups,patients in group C showed significantly higher walking speed,less asymmetry of step length and single support time,and milder stress response.The maximum hip extension angle,maximum knee joint dorsiflexion angle during support phase,FMA score,and the ADL score were significantly higher in group C than those in the other two groups.Conclusion Scalp acupuncture combined with lower limb rehabilitation robot can significantly increase the walking speed of stroke patients,thereby improving the patients'gait pattern and ability of daily living.
