机器人腹腔镜结肠癌手术的短期疗效观察

摘　　要：为比较机器人腹腔镜手术和传统腹腔镜手术治疗结肠癌的短期疗效,回顾性分析甘肃省人民医院结肠癌手术数据库中85例患者临床资料。按手术方式不同分为机器人组26例和腹腔镜组59例,机器人组采用机器人辅助腹腔镜下结肠癌切除术,腹腔镜组采用传统腹腔镜下结肠癌切除术。比较两组术后短期疗效及手术指标。结果显示,两组手术时间、术中出血量、淋巴结清扫数量和总引流量比较,差异无统计学意义(P>0.05)。机器人组平均住院时间、首次进食时间、流质饮食时间和通气时间均优于腹腔镜组,总费用高于腹腔镜组,差异有统计学意义(P<0.05)。结果表明,机器人腹腔镜手术的短期疗效优于腹腔镜手术。同时需要进行大样本多中心前瞻性研究来确定机器人辅助系统在结肠癌手术中的优势。This study was to compared the short-term effect of robotically performed laparocopic procedure(RPLP)and routine one for colonic cancer,retrospectively reviewed the data of 86 cases underwent surgery in Gansu provincial hospital,according to their different procedures,divided them into robot group(26 cases)and laparoscope group(59 cases):the former received robotically performed laparoscopic resection,meanwhile,the latter received routine one(i.e hand-assisted one perform);then,compared both groups short-term effect and operating norms.As results,in operating time,intraoperative bleeding volume,the numbers of lymph nodes removed,and total drainage volume there was no statistical difference between the two groups(P>0.05);but in the time for average hospitalization,for first eating,for eating fluid diet,and for purge,as well in total medical cost the former was all superior to the latter(all,P<0.05).Results show that short-term outcome of RPLP is more superior to routine one,but pending large-scale multi-center prespective researches to certify RPLP systme’s advantages in the treatment of colonic cancer.
