基于现代中医临床思维的扶正祛邪中药治疗大肠癌肝转移患者临床疗效Meta分析

摘　　要：目的:系统评价基于现代中医临床思维的扶正祛邪中药治疗大肠癌肝转移患者临床疗效的有效性和安全性。方法:全面搜集扶正祛邪相关的中药组方治疗大肠癌肝转移的临床随机对照试验(RCT),由2位研究人员独立筛选文献、提取资料、评价偏倚风险后,采用Revman 5.3软件对近期临床疗效、体力状况(KPS)、生存率、癌胚抗原(CEA)、肿瘤标志物199(CA199)、甲胎蛋白(AFP)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、生活质量评分进行Meta分析。结果:共纳入文献10篇,572例患者,其中对照组(常规治疗)271例,治疗组(扶正祛邪中药联合常规治疗)301例。与对照组比较,治疗组改善患者近期临床疗效、KPS、ALT、AST、生存质量评分及降低CEA、CA199的效果更佳,差异均有统计学意义(P<0.01)。结论:基于现代中医临床思维的扶正祛邪中药治疗大肠癌肝转移患者能提高其近期临床疗效,改善患者KPS和生活质量,降低肿瘤标志物CEA、CA199,且能改善肝功能指标ALT和AST。Objective:To systematically evaluate the effectiveness and safety of the clinical effect of Chinese herbs of reinforcing healthy qi and eliminating pathogenic factors for patients with liver metastasis of colorectal cancer based on clinical thinking of modern Chinses medicine.Methods:The clinical randomized controlled trails(RCT)of liver metastasis of colorectal cancer treated with Chinese medicine prescription of reinforcing healthy qi and eliminating pathogenic factors were comprehensively collected.Two researchers independently selected literature,extracting data and evaluating the risk of bias.Then,Revman 5.3 software was used for Meta-analysis on recent clinical curative effect,Karnofsky performance status(KPS),survival rate,carcinoembryonic antigen(CEA),tumor marker 199(CA199),alpha-fetoprotein(AFP),alanine aminotransferase(ALT),aspartate aminotransferase(AST)and the score of quality of life.Results:There were 10 literature included,involving 572 patients,271 in the control group(conventional treatment)and 301 in the treatment group(Chinese herbs of reinforcing healthy qi and eliminating pathogentic factors combined with conventional treatment).Compared with that in the control group,the treatment group had better effect on improving recent curative effect,KPS,ALT,AST and the score of quality of life,and decreasing CEA and CA199,differences being significant(P<0.01).Conclusion:In the treatment of liver metastasis of colorectal cancer,Chinese herbs of reinforcing healthy qi and eliminating pathogenic factors based on clinical thinking of modern Chinese medicine can improve the recent clinical effect,physical strength,quality of life,and the liver function indexes including ALT and AST,and reduce tumor markers including CEA and CA199.
