摘  要:目的探讨颅脑模型结合侧脑室模拟穿刺网络教学系统在神经外科临床教学中的应用效果。方法选取2018年12月至2019年12月在本科室学习的同一年级年轻医师100人作为研究对象,随机分为A、B两组,每组50人(每人学习时间为1个月)。A组为常规培训组,主要以带教讲解、学员观看为主的传统教学模式。B组为新型线上侧脑室穿刺教学组,利用计算机三维重建技术建立颅脑-侧脑室三维虚拟模型,对B组进行侧脑室解剖及穿刺相关理论教学,利用自主开发的基于个人电脑终端的侧脑室模拟穿刺网络教学系统对B组进行侧脑室穿刺训练及考核,并应用系统记录的培训数据分析教学效果及制定个体化教学方案。最后两组医师在实体模型上进行模拟穿刺,随后进行理论考试、实践技能考试及问卷调查,比较两组间学习效果。结果B组学习效果评分:理论考核成绩(85.7±6.9)分、实践技能考核成绩(94.6±7.2)分;问卷调查结果:理论掌握程度(4.12±0.73)分、操作掌握程度(4.25±0.68)分、信心及兴趣的提升(4.33±0.72)分、对教学方式的评价(4.52±0.66)分,均明显高于A组,差异有统计学意义(P<0.05)。结论利用基于模拟穿刺网络教学系统培训的新型教学方法效果显著,可快速提升年轻医师穿刺技术的熟练度和掌握度,激发其兴趣并增强其操作自信。Objective To explore the efficiency of online training system of lateral ventricle puncture based on 3D craniocerebral model in clinical teaching of neurosurgery.Methods One-hundred young doctors in the same grade who studied in our department from December 2018 to December 2019 were selected as the study subjects.They were randomly divided into two groups with 50 students each in a one-month period.In the traditional training group,students followed the instructions of the teacher to watch the whole process of lateral ventricle puncture.In the new teaching model group,students were trained and assessed by online training system of lateral ventricle puncture based on 3D craniocerebral model.Students could simply use a personal computer to learn and the training data recorded by the system were used to analyze the teaching effect and formulate the individualized teaching scheme.The two groups performed lateral ventricle puncture on the solid model afterwards.Finally,examination of clinical theory and clinical practice as well as survey were conducted to compare the learning effect between the two groups.Results The clinical theory score(85.7±6.9)and clinical practice score(94.6±7.2)evaluated by the examination in the new teaching model group were significantly higher than those in the traditional training group(P<0.05).The theoretical mastery degree(4.12±0.73),operational mastery degree(4.25±0.68),confidence and interest(4.33±0.72),and the satisfaction of teaching method(4.52±0.66)evaluated by the survey in the new teaching model group were also significantly higher than those in the traditional training group(P<0.05).Conclusion Online training system of lateral ventricle puncture based on 3D craniocerebral model is an effective teaching method.It can quickly improve the proficiency and mastery of lateral ventricle puncture of young doctors,stimulate their interest and enhance their selfconfidence.
