摘 要:目的探索典型病例引导PBL教学模式在外科学见习中的运用。方法将2019年3月1日-2019年12月31日见习的120名学生作为研究对象。按照随机分组方法,将其分成对照组和观察组,每组各60名。对照组采用传统的教学模式,观察组采用典型病例引导PBL教学模式。见习结束后,通过理论考试、问诊考试以及调查问卷的形式对典型病例结合PBL教学模式在外科学见习中的运用效果进行评价。结果观察组学生的理论考试总成绩及其中的病例分析题成绩均好于对照组,差异有统计学意义(P<0.001),选择及简答题成绩两组未见明显差异。问诊考试成绩和最终见习总成绩统计时,观察组也明显优于对照组,两组比较差异有统计学意义(P<0.001);通过调查问卷的结果显示,超过90%的学生表示,典型病例引导PBL教学模式在外科学见习改革中能够提高学习兴趣、牢固掌握知识能力、临床难题分析及自主学习能力。结论采用典型病例引导PBL教学方式可提高外科学见习医师的临床实践能力,帮助他们建立科学的临床思维构架,培养见习生的自主学习兴趣,是一种较好的外科学见习教学方法。Objective To explore the application of the typical case-guided PBL teaching model in surgical internship. Methods A total of 120 students who took part in surgical internship study from March 1, 2019 to December 31, 2019 were enrolled in this study. According to the random grouping method, they were divided into a control group and an observation group, with 60 in each group. The control group adopted the traditional teaching mode, and the observation group were performed the typical case-guided PBL teaching mode. After the whole internship, the effects of typical case-guided PBL teaching model in surgical internship were evaluated in the form of theoretical examinations, clinicalinterview and examinations and questionnaires. Results The total scores of theoretical examinations and the scores of the case analysis questions in the observation group were better than those in the control group. The difference was statistically significant(P<0.001). In the clinicalinterview and examination results and final trainee total score statistics, the observation group was also significantly better than the control group, the difference between the two groups was statistically significant(P<0.001).The results of the questionnaire showed that more than 90% of the students said that the typical case-guided PBL teaching model can improve the interest inlearning, the ability to master knowledge, the analysis of clinical problems and the ability to learn autonomously in the reform of surgery internship. Conclusion Using the typical case-guided PBL teaching method can improve the clinical practice ability of surgeon trainee doctors,help them establish a scientific surgery clinical thinking framework,and cultivate the self-learning interest of trainees, which is a better method of surgeon trainee teaching.
