医用射线防护喷剂防治放射性口腔黏膜炎的临床观察

摘　　要：目的观察医用射线防护喷剂(伯格曼)对头颈部肿瘤患者放射性口腔黏膜炎的防治效果。方法回顾分析头颈部恶性肿瘤放疗患者82例,其中42例仅给予康复新液口服及漱口处理的为对照组,40例在对照组基础上同时给予伯格曼喷于口腔黏膜表面的为试验组。两组均采用的常规分割调强放疗,肿瘤临床靶区(CTV)剂量为65~70 Gy/36次。结果两组患者放疗时均发生了急性口腔黏膜炎,77.5%的试验组患者放射性口腔黏膜炎出现在放射剂量大于40 Gy时,而对照组69.05%出现在40 Gy之前(P<0.001);试验组口腔黏膜炎严重程度(2、3级放射性口腔黏膜炎的发生率)明显低于对照组(P=0.005);放疗前后试验组患者的体重减轻程度明显低于对照组,有统计学意义(P=0.021)。结论预防性应用医用射线防护喷剂(伯格曼)可以推迟急性放射性口腔黏膜炎的发生,减轻口腔黏膜炎的严重程度,减少放疗期间体重的下降,防治口腔黏膜炎安全有效。Objective To observe the prevention and treatment effect of medical radiation protection spray(Bergman)on oral mucitis of head and neck tumor.Methods A retrospective analysis of 82 cases of patients with head and neck malignant tumor radiotherapy was performed.Among them,42 cases were treated with fufuxin solution orally and gargle as the control group,and 40 cases were treated with Bergman spray on oral mucosa surface based on the control group as the test group.Conventional segmental intensity modulated radiotherapy was used in both groups,and the clinical target area(CTV)dose was 65~70 Gy/36 times.Results Acute oral mucositis occurred in both groups during radiotherapy.77.5%of patients in the experimental group showed radiological oral mucositis when the radiation dose was greater than 40 Gy,while 69.05%of patients in the control group showed radiological mucositis before 40 Gy(P<0.001).The severity of oral mucositis in the test group(the incidence of grade 2 and grade 3 radioactive oral mucositis)was significantly lower than that in the control group(P=0.005).Before and after radiotherapy,the degree of weight loss in the test group was significantly lower than that in the control group,and there was statistical significance(P=0.021).Conclusion The preventive application of medical radiation protection spray(Bergman)can delay the occurrence of acute radioactive oral mucitis,reduce the severity of oral mucitis,reduce the weight loss during radiotherapy,and prevent and treat oral mucitis safely and effectively.
