胎儿心胸面积比测量在产前预测重型α-地中海贫血的价值

摘　　要：目的建立孕13周~26周正常妊娠心胸面积比的正常值,探讨胎儿心胸面积比测量在产前预测重型α-地中海贫血的价值。方法采用二维超声对1200例孕13周~26周正常妊娠和280例同孕周的α-地中海贫血高危妊娠胎儿进行心胸面积比测量,并分析比较测量值。结果在正常对照组,胎儿心脏面积、胸腔面积随着孕周增加逐渐增大,但胎儿的心胸面积比则基本恒定在25%~32%之间。280例高危组中发现86例胎儿心胸面积比的测量值比同孕周的正常对照组高(>35%),设定孕13周~26周心胸面积比率>35%为诊断异常临界值,以此诊断胎儿水肿的准确率为97.73%,灵敏度为100%。结论在孕早、中期测量胎儿心胸面积比是有价值的超声指标,可预测重型α-地中海贫血。Objective To establish normal 13~26 weeks of gestation mind area than normal pregnancy,fetal heart area ratio measurement in the value of prenatal prediction of heavyа-the Mediterranean anemia.Methods using two-dimensional ultrasound in 1200 cases of pregnancy and 280 cases of normal 13~26 weeks of gestation with gestational age ofа-the Mediterranean anemia of high-risk pregnancy fetal heart area ratio of measurement,and comparative analysis the measured values.Results in the normal control group,the fetal heart area and chest area gradually increased with the increase of gestational age,but the fetal cardiothoracic area ratio was basically constant between 25%and 32%.Among the 280 high-risk groups,the measurement value of fetal cardiothoracic area ratio of 86 cases was found to be higher than the average value of the normal control group at the same gestational week(>35%).The cardiothoracic area ratio of>35%at gestational week 13 to 26 was set as the critical value for the diagnosis of abnormal abnormalities,so the accuracy rate of fetal edema diagnosis was 97.73%and the sensitivity was 100%.Conclusion during the early and mid-measurement of fetal heart area ratio,ultrasonic measure of value,predictable heavyа-the Mediterranean anemia.
