摘  要:目的比较乳清蛋白与膳食纤维干预对妊娠糖尿病(GDM)患者血糖水平及营养状况的影响。方法将126例GDM患者,随机分为乳清蛋白组、膳食纤维组和对照组,各42例,并于三餐前30 min分别给予口服乳清蛋白、膳食纤维、安慰剂。比较3组患者干预前后空腹血糖、餐后2 h血糖(2hPBG)、血红蛋白、血清白蛋白水平,以及分娩方式、新生儿出生体重。结果干预后,乳清蛋白组和膳食纤维组空腹血糖、2hPBG水平均低于对照组,乳清蛋白组血红蛋白、血清白蛋白水平均高于其他两组(均P<0.05)。3组患者新生儿体重和分娩方式差异均无统计学意义(均P>0.05)。结论餐前口服乳清蛋白和膳食纤维饮食疗法均有助于控制GDM患者空腹血糖及2hPBG,且对母婴结局无明显影响,但乳清蛋白还可改善患者的营养状况。Objective To compare the effects of lactalbumin versus dietary fiber intervention on blood glucose and nutrition status in patients with gestational diabetes mellitus(GDM).Methods A total of 126 GDM patients were randomly divided into lactalbumin group,dietary fiber group and control group,with 42 cases in each group,and the three groups were orally given lactalbumin,dietary fiber,and placebo 30 minutes before three meals,respectively.FBG,2-hour postprandial blood glucose(2hPBG),hemoglobin,and serum albumin levels before and after intervention,as well as delivery approach and neonatal birth weight were compared among the three groups.Results After intervention,the levels of FBG and 2hPBG in the lactalbumin and dietary fiber groups were lower than those in the control group,the levels of hemoglobin and serum albumin in the lactalbumin group were higher than those in the other two groups(all P<0.05).There was no statistically significant difference in neonatal body weight or delivery approach among the three groups(all P>0.05).Conclusion Oral administration of lactalbumin or dietary fiber diet therapy before meals might contribute to FBG and 2hPBG control in GDM patients,and have no significant effects on maternal and neonatal outcomes,but lactalbumin can improve patients′nutrition status.
