内科住院医师规范化培训临床病例讨论探讨与实践

摘　　要：上海交通大学医学院附属仁济医院自2010年作为上海市国家级住院医师规范化培训基地以来,在住院医师规范化培训方面取得了一定的成效。内科病例讨论会——“砥‘例’前行”是仁济医院内科住院医师规范化培训中的一大亮点。每次病例讨论前由亚专业教学主任选定教学病例,撰写教案及简要病史并制作PPT,内科教学秘书提前一周在微信平台推送给住院医师做讨论准备,病例讨论期间骨干教师引导住院医师进行分析讨论,最后由教学主任总结并对相关疾病诊疗进展开展讲座。“砥‘例’前行”开展以来受到广大住院医师的青睐与好评,但病例讨论过程中依然存在一定问题,包括住院医师讨论积极性不高、教师教学能力不强、讨论形式单一等,这需要我们探索完善教学反馈、调动师资带教积极性,更重要的是培养内科住院医师主动分析思考的能力,从而为国家培养更多优秀的内科医师。Shanghai Renji Hospital has been a state-level standardized residency training base in Shanghai since 2010,where certain achievements have been made.Seminar of Internal medicine cases is the highlight in the standardized residency training in Renji Hospital.Before each seminar,the teaching director of internal medicine subspecialty would select the case,write teaching plans and brief medical history,and make PowerPoint presentation.The teaching secretary would deliver the case in the WeChat platform a week in advance for discussion preparation.During the seminar,a senior teacher would guide residents to analyze and discuss cases,and the teaching director would finally summarize the case and give lectures on the progress of diagnosis and treatment of the related diseases.Since the internal medicine case seminar was carried out,many residents have praised this kind of form.However,problems like lack of discussion enthusiasm,inadequate teaching ability and single discussion form exist in the process of case discussion,which need improvement.We need to constantly improve teaching evaluation and mobilize teachers’enthusiasm.What’s more important,we need to cultivate residents’ability of active analysis and thinking,so as to train more excellent physicians for our country.
