食管基底细胞样鳞状细胞癌中PD-L1表达及与临床病理的关系

摘　　要：目的探讨食管基底细胞样鳞状细胞癌(basaloid squamous cell carcinoma,BSCC)中PD-L1表达及与临床病理特征及预后的关系。方法收集37例原发性食管BSCC手术切除标本及对应癌旁正常组织,采用免疫组化SP法检测食管BSCC、癌旁正常组织中PD-L1的表达,并分析其与临床病理特征及生存预后的关系。结果PD-L1在食管BSCC中的高表达率为29.73%(11/37),在癌旁正常组织中不表达。PD-L1表达与食管BSCC肿瘤浸润深度、临床分期及有无淋巴结转移相关(P<0.05)。PD-L1在肿瘤浸润外膜层中的高表达率显著高于浸润至黏膜下层及肌层。Ⅲ+Ⅳ期食管BSCC中PD-L1的高表达率高于Ⅰ+Ⅱ期。PD-L1高表达患者淋巴结转移率高。Kaplan-Meier生存分析显示,PD-L1高表达食管BSCC患者的术后总生存期(P=0.0003)和无进展生存期(P=0.006)比PD-L1低表达患者显著降低。结论PD-L1是影响食管BSCC患者预后的独立因素之一,PD-L1高表达患者有望受益于肿瘤免疫治疗。Purpose To investigate the relationship of programmed cell death-ligand 1(PD-L1)in esophageal basaloid squamous cell carcinoma(BSCC)and its clinicopathological characteristics and prognosis.Methods The expression ofPD-L1 was detected by immunohistochemistry in 37 cases of primary esophageal BSCC and adjacent normal tissues,and its relationship with clinicopathological features and survival prognosis was analyzed.Results The overexpression rate of PD-L1 in esophageal BSCC was 29.73%(11/37),but it was not expressed in normal tissues.The overexpression of PD-L1 in esophageal BSCC was correlated with the depth of tumor invasion,clinical stage and lymph node metastasis(P<0.05).The overexpression rate of PD-L1 in the tumor infiltrating esophageal adventitia was significantly higher than that in the submucosa and muscular layer.The overexpression rate of PD-L1 in stageⅢ+Ⅳesophageal BSCC was higher than that in stageⅠ+Ⅱ.Patients with high expression of PD-L1 had high lymph node metastasis rate.Kaplan-Meier analysis showed that the total survival time(P=0.0003)and progression free survival time(P=0.006)of patients with high PD-L1 expression were significantly lower than those of patients with low PD-L1 expression.Conclusion PD-L1 is one of the independent factors that affect the prognosis of patients with esophageal BSCC.PD-L1 positive patients are expected to benefit from cancer immunotherapy.
