手术室模拟急诊心包填塞情景应急演练的效果分析

摘　　要：目的探讨手术室模拟急诊心包填塞情景应急演练的效果,提高各学科医护人员的应急协助能力及专科理论知识及技能。方法 2019年10月手术室联合麻醉科、心脏外科、体外循环科等医护人员组成手术室模拟急诊心包填塞情景应急演练小组,共14人,制定剧本模拟接诊心包填塞急诊手术情景应急案例演练,并对演练案例进行总结分析,提出改进与完善的方案。演练前后比较手术室护士理论、技能考核成绩,了解14名参演人员以及17名观摩护士对应急演练的满意度。结果演练前后手术室护士理论、技能考核成绩比较,均P<0.01,差异具有统计学意义,演练后理论及技能操作成绩均高于演练前。演练后各专科医务人员(包括参演以及观摩人员)对急诊心包填塞应急情景演练的满意度均在83.87%以上。结论情景模拟应急演练有利于提高手术室护士掌握急诊心包填塞病人手术护理和心肺脑复苏的理论知识、急救技能以及应急处理能力,同时提高医护团队协作能力,值得临床推广应用。Objective To investigate the effect of scenario simulation drills on emergency cardiac tamponade in the operating room so as to improve the emergency assistance ability,specialized theoretical knowledge and skills of medical staff in various disciplines.Methods In October 2019,the surgical anesthesia center combined with the medical teams from departments of cardiac surgery and extracorporeal circulation jointly established a scenario simulation team of 14 nurses and doctors for simulated emergency cardiac tamponage in the operating room.A script for the scenario simulation drills was written,followed by summing up based on case study and putting forward schemes for improvement.The theory and skill assessment scores of operating room nurses were compared before and after the drills and meanwhile the satisfaction of 14 participants together with another 17 nursing observers was investigated as well.Results The theoretical and skill operation scores after the rehearsal were significantly higher than those before the rehearsal(P<0.01).After the drills,the satisfaction level of the participants and nursing observers was over 83.87%.Conclusions The emergency scenario simulation drills can help operating room nurses master the theoretical knowledge of surgical care for critical patients and cardiopulmonary and brain resuscitation,improve their first-aid skills,enhance emergency treatment ability.Meanwhile the drills can improve the ability of medical team cooperation.It is worthy of clinical promotion and application.
